AFFIDAVIT

I, EDUARDO PUNGTILAN ULEP, of legal age, Filipino citizen, and a resident

of Brgy. 17, San Nicolas, Ilocos Norte, Philippines, after having been duly sworn to in
accordance with law hereby depose and states that;

1. I am the surviving heir being the biological son of the late EMILIO ULEP
who is the owner of a parcel of land designated as Lot No. 15095 (S) Cad-
307-D located in Brgy. 17, San Nicolas, Ilocos Norte, Philippines;

2. I am currently applying for the installation of water connection with the San
Nicolas, Water District (SNWD) in the above-mentioned property for personal
use and consumption;

3. I am presently occupying the above-mentioned land along with my family;

4. The above-mentioned property was acquired by way of inheritance;

5. To the best of my knowledge the above-mentioned property is free from liens

and encumbrances;

6. In case somebody contest the said water connection, I am willing to let the
San Nicolas Water District (SNWD) immediately the water service
connection; and

7. That I am executing this affidavit to attest to the veracity of my statements and

for whatever intents and purposes it may lawfully serve.

IN WITNESS WHEREOF, I have hereunto set our hands this 11 th day of
