7.1.6 The QA&CU will:

7.1.6.1 Conduct ‘Spot check’ safety inspections on marine vessels in the field.

7.1.6.2 Monitor Contractor Safety Performance through the on-line ‘e-system’ by monitoring
contractor performance in:

Marine Incidents
Near Miss Reporting
Closure of Incident Recommendations
Vessel Safety Inspection Deficiency Numbers
Closure of Vessel Safety Inspection Deficiencies

7.1.6.3 Report on Marine Contractor Safety Performance on a regular basis to the concerned
Division Head, Marine Manager through:

Marine Monthly Safety Statistics Report


SOC (Safe Operations Committee) Meetings

7.1.6.4 Manning Contracts: In the event of acceptance, the report is dispatched to the Marine
Contracts Group with copies forwarded to concerned Operating Division
Superintendent. Crew Review, Evaluation and Monitoring of Contractor Officers will
be as per MIM 1200.003.

7.2 VESSEL SUBSTITUTIONS

7.2.1 For New Contracts with Attachments I and III to Schedule “B” deductions:

7.2.1.1 Contractor will coordinate with the Company Representative prior to submission
of the proposal letter for vessel substitution to VAT.

7.2.1.2 Contractor will submit a proposal letter for vessel substitution addressed to the
Marine Manager along with the specifications of the proposed substitute vessel
properly filled up as per Attachment I to schedule B of the respective contract.
The Contractor’s request is to include the reason for substitution and duration.

7.2.1.3 Contractor will send a copy of the proposal letter request to the VAT and the
concerned Company Representative for their evaluation.
