It has been noticed that your performance as Sales Support Assistant is not at par with the
acceptable level. This has been confirmed with the delays on duties and assigned task given to
you since the start of this year. Some of which are:

1. Failure to monitor and filing of TS documents which resulted to delay in crediting of


commission. Some Trade Marketing Coordinators need to shell out from their own
pocket for the commission be released on time.
2. Failure to monitor proper liquidation of consumer TMC. Due to neglect, submitted
liquidation were tagged as unliquidated and were deducted to payroll of TMC.
3. Frequent absences is also observed which hamper productivity of the team.

With the above-mentioned undelivered tasks, your actions may constitute several
violations on our revised GAPP which are as follows:

1. Failure to submit required and accurate reports on time which constitute MINOR
OFFENSE
2. Insubordination under the following grounds; which may constitute LESS GRAVE TO
GRAVE OFFENSE.
a. Failure, neglect or refusal to carry out verbal or written job instructions.
b. Gross and habitual neglect of assigned tasks or duties.
c. Willful disobedience.

Combining all the possible offenses mentioned above, it may constitute WILLFUL
DISOBEDIENCE TO LAWFUL ORDERS and/or GROSS AND HABITUAL NEGLECT OF DUTIES which
defines as the want or absence of or failure to exercise slight care or diligence, or the entire
absence of care. It evinces a thoughtless disregard of consequences without exerting any effort
to avoid them. This offense is a ground for DISMISSAL or TERMINATION from service.

Your acts may constitute not only a violation of the above-quoted provision but may
likewise amount to insubordination in accordance with the provisions of the Labor Code.

You are, therefore, given five (5) calendar days from receipt hereof to explain in writing
why no disciplinary action should be taken against you, including possible termination from
service, for the acts earlier described.

Moreover, your refusal to submit a written explanation within the time provided herein
will be construed as a waiver of your opportunity to be heard and explain your side. The
Company will then proceed with its investigation and shall render its decision based on the
documents at hand and evidence gathered.

Please give this matter your most utmost attention.
