EXTRAJUDICIAL SETTLEMENT OF ESTATE

Know all men by these presents:

This AGREEMENT is entered into by, between and among:

REYNALDO B. MERCADO, of legal age, Filipino, widow and with

residence at 875 Macopa St., Malaria Compound, Tala, Caloocan City;

RHYAN E. MERCADO, of legal age, married, Filipino, and a resident of

875 Macopa St., Malaria Compound, Tala, Caloocan City; and

JAMIE ROSE E. MERCADO, of legal age, single, Filipino and a resident

of 875 Macopa St., Malaria Compound, Tala, Caloocan City;

WITNESSETH:
Whereas, we are the sole heirs of NENITA E. MERCADO, who died on
April 12, 2021, at Dr. Jose N. Rodriguez Memorial Hospital and Sanitarium, Tala,
Caloocan City. Attached as ANNEX A is a copy of the Death Certificate of Nenita
E. Mercado;

Whereas, the deceased, Nenita E. Mercado, died intestate, without Will or

Testament, and without any outstanding debts in favor of any person or entity;

Whereas, Nenita E. Mercado left a bank deposit at Union Bank

Novaliches, Quirino Highway under Savings Account No. 109561648128;

NOW THEREFORE, Pursuant to Rule 74, Sec. 1 of the Revised Rules of

Court of the Philippines, and being with full capacity to contract, we do hereby
adjudicate equally unto ourselves the proceeds or amount of money left or
creditable to us in said account.

We hereby affirm that the foregoing was knowingly executed of our own
voluntary will, without force, intimidation or violence upon our persons.

ACKNOWLEDGMENT
BEFORE ME, a Notary Public for and in the City of Quezon this ___ day
of April 2021, personally appeared herein parties and exhibited their respective
competent evidence of identity, to wit: Reynaldo B. Mercado, his Senior Citizen’s
I.D. with Control No. N-38076; Rhyan E. Mercado, his Professional Driver’s License
No. N25-00-081782; and Jamie Rose E. Mercado, her PRC ID Card with
Registration No. 0570565, all known to me to be the same persons who executed
the foregoing Deed and they acknowledged to me that the same is their free act
and deed for the purpose herein stated.

This instrument refers to an Agreement consisting of two (2) pages

including that on which this acknowledgment is written and was signed by the
parties and their instrumental witnesses on each and every page.
