OCCUPATIONAL HEALTH AND SAFETY ACT, 85 OF 1993 GSR 3(1) and (4) –

FIRST AIDER APPOINTMENT

I, COREY TAYLOR, the 16.2 appointee of ABC Scaffolding, hereby appoint you, LAYNE STAYLEYas the GSR 3(4),
Level 3 First Aider for ABC Scaffolding and will be situated on the Gauteng Santon Convention Centre project.

In terms of this appointment, the following will be deemed to be your functions:

1. To ensure that the first aid box or boxes under your control remain properly stocked to meet all foreseeable incidents
that may occur in your designated area of responsibility;
2. To ensure that first aid boxes are properly safeguarded, that the positions of such boxes are properly demarcated and
that the names of first aiders are conspicuously displayed on, or at, first aid boxes;
3. To ensure that you keep your first aid certificate current at all times for the duration of this appointment;
4. Should the activities in your area of responsibility involve the use of hazardous chemical substances, you must
familiarise yourself with the material safety data sheet (MSDS) for the particular substance/s, with particular reference
to the first aid measures to be applied in case of an emergency;
5. To be readily available during working hours/shifts to administer first aid when required; and
6. You will be required to undergo training in order to obtain a valid certificate of competency in first aid.
The above should be read in conjunction with GSR 3 in its entirety.
