RISK ASSESSOR APPOINTMENT

FRED DURTS
I, COREY TAYLOR, the 16 (2) appointee of ABC SCAFFOLDING hereby appoint you FRED DURST, as
risk assessor for the Sandton Convention Center Project.

In terms of this appointment, you are required to ensure that the necessary risk assessments are carried out
before the commencement of, and during, construction work. The mentioned risk assessments shall include at
least the following:

1 The identification of the risks and hazards to which persons may be exposed;
2 The analysis and evaluation of the risks and hazards identified;
3 A documented plan of safe work procedures to mitigate, reduce or control each of the identified risks
and hazards; and
4 A monitoring-and-review plan.
5. You are further required to ensure that copies of the latest, updated risk assessments are available, on-
site, for inspection and that the and the Construction SUPRVISOR are in possession of such risk
assessments.
6. Any problems which you may experience in the execution of this appointment must be reported to the
construction manager

This appointment is valid from 0/0/0 to the completion of the stipulated construction work.
