AFFIDAVIT OF LATE REGISTRATION

I, CARLA S. DE JESUS, of legal age, Filipino, and a resident of La Carlota City,

Negros Occidental, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and say that:

1. I am the same Carla S. De Jesus who is the petitioner in the case docketed as Civil
Case No. 18-2656 for Declaration of Absolute Nullity of Marriage before the
Honorable Regional Trial Court of Branch 43, which became FINAL and
EXECUTORY on June 5, 2023.

2. The Regional Trial Court Branch 43 issued the decision declaring my marriage
null and void on September 30, 2022.

3. I received the copy of the said decision on October 7, 2022, and at that time, I
was in the USA working as a teacher. I only arrived in the Philippines last June
27, 2024.

4. It was only when I arrived in the Philippines last June 27, 2024, that I had the
chance to register my papers before the Local Civil Registrar and the Philippines
Statistics Authority.

5. Hence, I am now causing the registration of the Decision and Finality of

Judgment dated September 30, 2023, with respect to my annulment.

6. I am executing this affidavit to attest to the truthfulness of the foregoing

statement, and for the late registration of my annulment before the Local Civil
Registrar and the Philippines Statistics Authority, and for whatever legal
purposes this may serve best.
