Dear TRUIST client:

As a follow-up to our telephone conversation, this is to confirm the initiation of a review of the
transaction(s) on your TRUIST account that you have reported as being fraudulent/unauthorized.
As indicated in our conversation, an Electronic Error Resolution form, ACH written statement,
affidavits and/or other dispute forms are included for your completion and /or signature, based on
your type of claim as initially described to TRUIST. Please note that affidavits do require signatures in
the presence of a notary, and are your sworn statement under oath that the matters set forth on the
affidavit are true. The enclosed documents, in addition to any other documentation we may have
requested, must be returned to us within 10 business days of your receipt. For clients calling in a
claim from other than a TRUIST location, also enclosed is a postage-paid envelope that you may
use for the return of these documents. Failure to complete, sign and return the enclosed documents
may result in our inability to complete the investigation on your case and could result in the reversal
of any credits that may have been applied to your account as a result of this claim.

A TRUIST fraud claims specialist may be contacting you in order to interview you and gather
additional information, or if there is a need for you to provide additional documentation. In order to
establish the validity of the call, the specialist will identify themselves as a TRUIST employee and
will reference the claim number provided above. You should never discuss your case with anyone who
is unable to provide this information. Careful consideration will be given and a decision rendered and
communicated to you as quickly as possible after we have collected all relevant information and
conducted all necessary research.
