Activity Title Conduct of 2nd Quarter School Monitoring, Evaluation and


Adjustment
IDENTIFYING INFORMATION
Venue Bolocboloc Elementary School
Objectives * Conduct the 2nd quarter School Monitoring Evaluation and
Adjustment for this school year;
*Accounting the targets and accomplishments of teachers for
the 2nd quarter;
*Checking and validating the necessary information and
insights on the status, progress, and results of the delivery of
basic education in schools concerning access, quality,
governance and accomplishment;
*Give technical assistance on some issues and gap of teachers
on certain KRAs; and
*Finalized and submit the accomplish SMEA.

Activity Description This is a half-day Face-to-Face gathering of all teaching and
non-teaching personnel of Bolocboloc ES.
The activity started at exactly 1:00 in the afternoon.
The activity was noted as successful as all objectives were
achieved
Highlights * Each teacher was given a copy of the SMEA forms to filled up
and accomplished based on their evidences and MOVs
presented.
*Giving of technical assistance on some issues and gap of
teachers on certain KRAs including ICT.
*Accomplished and submission of the SMEA form.
RESULTS
Achievement of *All Teachers were able to accomplished and submit the SMEA

concerning issues and gaps in access, quality, governance in

Policy Recommendations  Crop a localized rewards and recognition guidelines
 Deliberate and collaborate with SDO representative,
District Supervisor, School Planning Team (SPT),
Teachers, Guidance Counselors, School Governing
Council (SGC), GPTA Officers, Barangay Officials,
Stakeholders, Government Officials, etc. who has direct
control to resolve the issues/concerns/gaps/problems of
the participants shall then act by providing possible
solutions/recommendations to the
issues/concerns/gaps/problems reflected in the
presentation of the school.
 Allocate budget in Annual Procurement Plan (APP) for the
SMEA
