I, Zahoor Ahmad Gilkar F/o Mohammad Hamza Gilkar R/o Naibasti, Anantnag do hereby solemnly affirm
and declare on oath as under:-

1. That I am permanent resident of Jammu and Kashmir UT.

2. That my ward is holding a Passport bearing No. S7417044.
3. That mistakenly i have admitted my wards name as Mohammad Hamza instead of Mohammad
Hamza Gilkar.
4. That now I intend to get the name of my corrected in the records of Passport office Srinagar as
Mohammad Hamza Gilkar instead of Mohammad Hamza for which I hereby enclose relevant
documents as proof.
5. That I hereby declare that statement tendered above is true and correct, in case proved to be
fake/false at any stage I shall personally be responsible for consequences.

Mohammad Asad Gilkar instead of Mohammad Asad for which I hereby enclose relevant
documents as proof.
5. That I hereby declare that statement tendered above is true and correct, in case proved to be
fake/false at any stage I shall personally be responsible for consequences.
