departure day at 2nd Floor Division Office of Agusan del Norte . It is considered that our son/daughter
will be benefited from his/her participation in these competitions.
As parent, we have mentored my child to follow rules and safety precautions. If my child will not
follow rules and safety precautions that may result in injury or damage to property, I understand
that the school has made its very best to ensure the security and welfare of my child.

akong anak. Isip ginikanan, akong gitudloan akong anak sa pagsunod sa mga bala-odnon. Sa higayon
