PROGRAM DESIGN
I. TITLE:

Non-Communicable Disease (NCD) Risk Assessment Program for Local Government


Unit Employees and Barangay Officials.

II. INTRODUCTION:

Non-Communicable Diseases (NCDs) are a significant public health concern globally,


contributing to a high burden of morbidity and mortality. Local Government Unit (LGU)
Employees and Barangay Officials play pivotal roles within their communities and face
various occupational stresses that may impact their health. Implementing an NCD Risk
Assessment program can help identify individual health risks, promote healthy behaviors,
and prevent the onset of NCDs.

III. RATIONALE:

The prevalence of NCDs, such as cardiovascular diseases, diabetes, and cancer,


continues to rise, posing challenges to both individuals and healthcare systems. By
conducting NCD Risk Assessments among LGU Employees and Barangay Officials, we
can proactively identify modifiable risk factors, provide targeted interventions, and
empower participants to make positive lifestyle changes to reduce their risk of developing
NCDs.

IV. GOALS AND OBJECTIVES:

 To assess the NCD risk profiles of Local Government Unit Employees and Barangay
Officials through comprehensive risk assessments.
 To raise awareness about modifiable risk factors associated with NCDs and promote
healthy lifestyle behaviors.
 To provide personalized recommendations and resources to support individuals in
reducing their risk of NCDs and improving their overall health and well-being.
V. TARGET RECIPIENTS:

All LGU Employees and Barangay Officials are the primary target recipients of this
program. By engaging this diverse group of individuals, the program aims to reach a
broad demographic and promote a culture of health and wellness within the community.

VI. RESOURCES NEEDED:

 NCD risk assessment tools (questionnaires, health screenings, biometric


measurements)
 Educational materials on NCD prevention, healthy lifestyle practices, and risk factor
management
 Health professionals for conducting assessments, interpreting results, and providing
counseling
 Collaborations with local healthcare facilities or providers for referrals and follow-up
care

VII. FUNDING SOURCE:

The funding for this program could be sourced from various avenues, including:

 GAD Non-Communicable Disease Program

VIII. RECOMMENDATIONS:

 Establish a program committee comprising representatives from the LGUs, healthcare


professionals, and community leaders to oversee program planning and
implementation.
 Conduct awareness campaigns and educational sessions to engage participants and
encourage active participation in the NCD Risk Assessment program.
 Monitor and evaluate program outcomes to assess effectiveness, identify areas for
improvement, and ensure sustainability through ongoing support and follow-up.
