FRONTLINE SERVICES
I. PROSECUTOR’S CLEARANCE / CERTIFICATION
Requirements:
 Personal appearance of the securing party
 Community Tax Certificate or any valid ID
 Documentary Stamp

Period when to avail : MONDAY TO FRIDAY 8 AM TO 5 PM

II. ADMINISTRATION OF OATH

III. RECEIVING OF COMPLAINT

Note : It must be filed within FIFTEEN (15) DAYS upon receipt of the
Resolution otherwise EITHER the necessary Information will be filed on the
proper court (Probable Cause findings) OR the case will be considered
dismissed.
5.2. Resolution of Motion for Reconsideration / Appeal finding probable cause

Procedure of complaint in filing complaints against OPP employees : Submit written


complaint to the Office of the Provincial Prosecutor of Ilocos Sur, Vigan City (Main Office) and it will
be forwarded to the Regional Prosecutor’s Office, San Fernando City, La Union for proper
investigation.
