USA Form Guide

You should have the following documents available while you complete your DS-160:

 Passport
 Travel itinerary, if you have already made travel arrangements.
 Dates of your last five visits or trips to the United States, if you have previously travelled to the
United States. You may also be asked for your international travel history for the past fiveyears.
 Resume or Curriculum Vitae - You may be required to provide information about your current
and previous education and work history.
 Other Information - Some applicants, depending on the intended purpose of travel, will be
asked to provide additional information when completing the DS-160.

Once review is complete click “SUBMIT” to submit the application. After you click on submit no
changes can be made to the application.

Save the DS160 Confirmation for your reference. The form is not required; however you can save
the copy of the same for your future reference.

SCHEDULING APPOINTMENT:

Click on “CREATE ACCOUNT”, enter your email id, name as on the passport and the password.
Once the account is created login to the account to select the payment option and schedule
appointment.

1. Click on New Application / Schedule Appointment

2. Select if applying for Immigrant OR Non-Immigrant visa

3. Select you location of residence


4. Select the language of interview
5. Select visa category

6. Select visa class

7. Review the information selected


8. Enter you passport details and the application number and all themandatory
information and click on continue.

9. Click on ADD BY NAME if family member applying withyou.


10. Provide the information asked.
11. Please select Courier option and kindly provide the address where you want the passport
to be couriered once it is ready for collection.

12. Select the option as NEFT – Bank of America for the fee payment. After selecting TIER
number will be generated (save the copy of the same)
13. You can also make the payment through NEFT option from your account directly. Please
check the bank options before proceeding with the NEFT payment option.
14. After the fees are paid the receipt number will be update automatically and you will also
receive an email confirmation for the same, after which you can continue and schedule
the appointment for the interview with the Counselor and for Bio-metrics.

15. Once the appointment is scheduled save the copy of the appointment confirmation
which will be required at the time of interview and bio-metrics

Documents required for Bio-metrics:

 Original passport valid for 6 months from the return date + oldpassport
 Appointment confirmation letter
 DS160 Confirmation copy
 2 photos (50mm x 50mm, white background, matt finish, 70-80% face showing)

Documents required at the time of Interview with the Counselor:

 Valid original passport. You are recommended to have passport valid for at least six months beyond your
interview date.
 CEAC Barcode printout (DS 160 Confirmation Page).
 Visa appointment letter printout.
 Invite letter copy from the US counterpart Address to the American Consulate General, Mumbai
/Chennai/Hyderabad/New Delhi/Kolkata (as applicable) – A valid invite letter from your host country is

 Covering letter from India Company Address to the American Consulate General,
Mumbai/Chennai/Hyderabad/New Delhi/Kolkata (as applicable) –Employees applying for business visas
would need to fill the draft https://inside.hr.amazon.dev/in/en/employment/getting-hr-help/employment-
verification.html and raise trouble ticket with ERC for visa covering letters prior to their Visa interview along

Apart from the documents mentioned in the checklist additional documents may be
requested. This is on case to case basis and solely at the discretion of the consulate.

7. Two photograph size should be 2 x 2 inches (51 x 51 mm) photographs [as per the standards

Granting of visa is entirely at the discretion of the Visa Officer, they may or may not check
all the documents mentioned above, however it is mandatory to carry them just in case you
are asked.

Photo specification:

 Photo must be 2 inches by 2 inches


 The height of the head (top of hair to bottom of chin) should measure 1
inch to 1 3/8 inches (25 mm - 35 mm)
 Make sure the eye height is between 1 1/8 inches to 1 3/8 inches (28 mm
– 35 mm) from the bottom of the photo

Post interview, US Visa stamping process takes minimum 3 to 4 Business working days.
