JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

(RE: DELAYED REGISTRATION OF BIRTH)

We, JASON NEPOMUCENO y BRONDIAL, of legal age, Filipino and a resident of

Purok 2, Brgy. San Francisco, Vicotria, Laguna and JAN REY DIOSO y ALVARO, also of
legal age, Filipino and a resident of Purok I Brgy. San Francisco, Victoria, Laguna, after having
been duly sworn to in accordance with law hereby depose and state that:

1. We know the facts and circumstances surrounding the birth of the child named JOAQUIN
CARLO CUSTODIO who was born on October 14, 2011 at OSPITAL NG CABUYAO,
BRGY. TRES, CABUYAO, LAGUNA to his biological parents JOSE CARLO
MANZANARES y NG and CHARISMA CUSTODIO y NUQUI, both being of our
personal acquaintances/neighbors;

2. The said child was born outside of wedlock;

3. The birth of the child was attended by DRA. MARIA CECILIA CASTELO, an OB-GYNE
at OSPITAL NG CABUYAO, BRGY. TRES, CABUYAO, LAGUNA;

4. The birth certificate of the said child was not immediately registered with the Office of the
Civil Registrar, CABUYAO, LAGUNA, primarily because of the inadvertence of the
parents or failure of the person(s) delegated to do so, hence, this Joint Affidavit;

5. The birth certificate of the child is not registered with any Office of the
Local/Municipal/City Civil Registrar;

6. We are executing this Joint Affidavit to attest to the truth of the foregoing facts of birth and to
support the application for delayed registration of the same with the Office of the Civil
Registrar, CABUYAO, LAGUNA.

IN WITNESS WHEREOF, we have hereunto set our hands this 2nd day of January, 2024 in
Victoria, Laguna, Philippines.
