AFFIDAVIT OF ADMISSION OF PATERNITY

I, JOSE CARLO MANZANARES y NG, of legal age, Filipino, with residence and
postal address at Purok 2, Brgy. San Francisco, Victoria, Laguna, after having been duly
sworn to in accordance with law, do hereby depose and state that:

1. I am the biological father of a male child named JOAQUIN CARLO CUSTODIO,

born to CHARISMA CUSTODIO y NUQUI, on OCTOBER 14, 2011 at the OSPITAL
NG CABUYAO, BRGY. TRES, CABUYAO, LAGUNA;

2. That at the time of his birth, I was not legally married to his biological mother;

3. That I hereby acknowledge and admit my paternity/filiation of JOAQUIN CARLO

CUSTODIO;

4. That I am giving my consent for my child to use my surname MANZANARES and

the same be reflected in his certificate of live birth;

5. I executed this affidavit in order to attest under oath the facts surrounding the
foregoing and for whatever legal purpose it may serve;

6. Affiant sayeth naught.
