Standard Terms and Conditions for the Supply of Laboratory Quality Products and Services

Introduction

International standard ISO 17043 define proficiency test as determining the performance of this program sets out the
general criteria for the implementation of technical proficiency test mode for Proficiency testing. The purpose of the
program is to evaluate the performance of the participating laboratories, based on objective evidence through the results
they produce and report to LGC as proficiency testing provider. The participation in this proficiency test is open to
laboratories worldwide.

Conducting the testing

The test must be conducted under normal routine conditions under test desired conditions and the technical responsible
must complete the results form. The laboratory will perform the measurements according to the test instructions.

Both the operators and the equipment used must perform the tasks under the normal routine established by each laboratory.
The technical manager of the laboratory must complete the results form.

The participant must ensure that the equipment used in conditions that operators are trained and qualified to avoid or
minimize potential sources of error.

The results obtained by the participating laboratory must be evaluated as being consistent or inconsistent with respect to
the reference values. Assigned values and statistical summaries for methods / testing procedures used by each group of
participants (if different groups of participants use different methods).

Confidentiality

To ensure that the results of the participant are known only by that laboratory, PT provider will be assigned a code number
along with the test samples and the return of results, in order to respect the confidentiality of data as required by our
procedure management system.

Final Report

The result of this program will be detailed in a report to be sent to each of the participating laboratories, respecting the
confidentiality procedures. The final report and certificate of participation will be sent to participants by email.

Fees and Payments Terms.

The participation fee of this proficiency testing is in Pakistan Rupees and detailed in the
last page of this agreement.

Shipping costs are included in the cost. Advanced Certification Pvt limited reserves the
right to not send samples or the report for previous samples to any participant that has not
paid whiting the terms stated on the invoice.

If for some reason, a participant withdraws from participating in the program once sent
the samples, must pay the full cost of the scheme. The forms of payment are Bank
transfer or by cheque in the name of title ADVANCED CERTIFICATION Pvt Ltd. The
Customer shall make all payments due under the Agreement without any deduction
whether by way of set-off, counterclaim, discount, abatement or otherwise.

The Customer shall pay the payment 100% advance payment. Order can be done by
issuance of purchase order and payment can be paid as per clients invoice clearance
procedure.

The PRA Sales Tax @16% will be added in the TAX INVOICE. If the 16% sales tax is
ducted by the client then sales challan need to submitted after/within one month of
invoice. If sales tax is paid to Advanced Certification, then we will submit the tax and
tax chalan will be sent to you within one month.

in addition to other amounts payable hereunder, any costs reasonably incurred

(including without limitation, legal costs and fees of debt collection agencies) in
recovering any amounts due from the Customer pursuant to the Agreement.
