Parental Consent form

Dear Parent,

Clubs and Societies at The City School provide ample opportunities to our learners to enhance soft skills
such as, leadership, tolerance, teamwork, courage, confidence and responsibility. These clubs and
societies transform our children into emotionally and socially strong person with deeper understanding
of different areas of life by adapting various life skills to achieve their future goals. We are pleased to
inform you that we are reincarnating our clubs and societies in our school with new ideas and skills to
prepare our learners for 21 st century life challenges. Therefore, we request your permission for your
child to participate in these clubs operating in the school premises as per the given schedule:

Days: Every Tuesday and Thursday Timings: 1:30 pm ____2:20 pm
