Raghunathpur, Uttarpara, Police Station – Uttarpara,
District – Hooghly, Pin code – 712247.

Subject: Legal cum Demand Notice for claiming the death

benefit and other benefit/s of Late Manoj Kumar Singh, who
was the permanent employee of National Insurance
Company and was employed as Cleaner at your good office.

Sir,

I have been instructed and I am duly authorized by My Client, to hereby

serve upon you this legal notice.

1. That My Client is the son and the legal heir of Late Manoj Kumar
Singh, (who died on 04/05/2002).

2. That said Manoj Kumar Singh was a permanent employee of

National Insurance Company, bearing Employee Number – 66817.
He was posted at NIC, Division – XVIII, Cal.

3. That my clients father joined as Cleaner of Office floor and toilet

etc. of your esteemed organization on 10th May’ 1989 with a Basic
Salary of Rs. 993/- (Rupees Nine Ninety Three only) (as on
01.07.1998) and Total Emolument of Rs. 1871.94/- (Rupees One
Thousand Eight Hundred Seventy One and Ninety Forty Paisa only

4. That my client’s father sincerely discharged his duty till his death.
My client’s father attached with your good office almost 10 years.

5. That after the death of Late Manoj Kumar Singh, you promised to
the family member of Late Manoj Kumar Singh and offer a job
from the family member of Late Manoj Kumar Singh at your good
office as per the qualification of the applicant. But it’s more than
20 years from the death of my client’s father, you failed to stay
with your words. It’s very unfortunate for my client as he is very
much interested to apply for the job, which was offered from your
side, at the time of death of my client’s father.

6. It is pertinent to mention that myself along with my family

member till date did not receive the amount from Provident Fund,
pension etc. and any other relief or reliefs as the legal claim of
Late Manoj Kumar Singh.

7. That my client belongs to a very poor and needy family and any
how he is maintaining his family but now is the position is that if
he does not get the death benefits of his deceased father, then my
family will die with starvation.

8. That my client also claims for a job as per your promise, failing
which, his family will be facing starvation and cannot live a
healthy life.

In view of the above as per My Client’s instruction, you are directed

to sit with my clients and settle the matter within 15(fifteen) days from
the date of receipt of this legal notice. Kindly note, that in the event of
your failure, My Client shall take appropriate legal steps, all of which
shall be undertaken at your cost & peril.
