1. Click on repeat medication on
the left hand side of the
screen
2. Click on the medication you
would like to request (you can
only request REPEAT
medications)
3. Click on request medication
4. If you need to add a comment
then enter in comment box
5. And click confirm request
6. And DONE
7. If your medication has been
rejected it will show on the
medication page

Step by Step Guide to access your
online
1. Fill out the online registration
form at the Northwick surgery
2. Reception will hand you an
online token.
3. Please visit
https://patient.emisaccess.co.
uk
4. And click on register online
button.
5. Enter Surgery postcode Ha1
2nu and click on the
Northwick surgery
6. It will ask you “have you
received a letter” click on
YES”

7. Enter Linkage details and
Account ID (found on
registration token)
8. Enter Details – please ensure
SURNAME is in CAPITALS
9. Enter email address that you
have given the surgery
10. Create password

account you will have access to

records 1. Click on home page and then next
 To view ALL of your medical to upcoming appointments –
records then please speak to “New Booking” ( you can book
appoitnemnt for the same day at
the surgery.
08.15am or at any time for future

2. Click on the appointment time slot
that is avaliable
 3. Enter reason you are seeing Dr
4. And click on Book Appointment
5. All appointment will show on
upcoming appointments in home
screen
