INTRODUCTION
The Health and Safety Plan is the main document for management of health and safety on site. It
should be prepared before construction work starts. However, it is a live and dynamic document
that will change and grow during the project and should be reviewed and updated on a regular
basis.

HEALTH SAFETY AND ENVIROMENTAL POLICY
STATEMENT
GENERAL
The Company shall plan and execute its operations (Civil, Electrical and Mechanical Worse)
in such a way as to
1. Ensure physical protection (Health and Safety) of its employees, Clients, Visitors, and
any third part that could be affected by its operations;
2. Ensure good relation and liaison, making life more meaningful and comfortable to the
inhabitants of the community in which they operate;
3. Ensure continuous attention to all environmental matters, elated to the activities
performed in order to prevent any possible environmental damage
4. Ensure the security of life and properties of its employees, those of the contractors’
visitors and members of the public;
5. Continually monitor its organizational, executive and technological effort for
improvement of all aspects regarding health, safety and environmental protection

A high safety level in the company contributes to the quality of its image. Every employee is
empowered to act in matters of health, safety and environmental protection. No accident or
damage to the environment can be justified.
The company undertakes to make personnel and management aware of its integration of HSE
matters in all work procedures and provide the necessary equipment and training in order to
reach its stated objectives.

DRUG AND ALCOHOL POLICY


The executive management the company warrants that it has a policy on drug and alcohol
abuse. Under this policy, alcohol impairment shall be defined as a blood-alcohol content of
40mg of alcohol per 100ml of blood or greater
All employees of the company may be subjected to test and screening at random and without
warning in addition to routine medical examinations
An objective of the policy is that the frequency of the random and unannounced testing shall
be adequate to act as an effective deterrent and that all employees be testing a minimum of
once yearly through either random testing or routine medical examination.
It is understood that an actual impairment or any test finding of impairment shall not in itself
mean that the company has failed to exercise clue diligence.
In addition, the company affirms that any imposed client’s drug, alcohol and search policy
general requirements shall be complied with.
ENVIROMENTAL POLICY
The environmental policy of company is to formulate and implement procedures which avoid
adverse impact on our environmental, and to mitigate those impacts, which are unavoidable.
This policy also strives to conserve and enhance, whenever possible, the resources of our
environment.
It is the Company’s policy to
1. Comply with all applicable Nigerian national environmental laws and regulations.
2. Maintain a self-monitoring programme to ensure continued compliance.

All levels of government in the promulgation and revision of environmental protection


regulations based on scientific facts that are economically and operationally feasible.
Assume that the expense of environmental protection is a legitimate cost of conducting
business in a modern society. Achieve environmental by employing the best control
mechanism, process and procedures, which have been proven technically sound and
economically feasible. The employees are to be aware of the impact of their assignments on
the environment and to assume responsibility for environmental protection at their level.

1.0 EMERGENCY RESPONSE PLAN

1.1 General
There are various problems and incidents, which could occur on site and which would
require urgent attention. Hence, the need to have an emergency procedure on the site.

The Centre point of Contractor’s emergency procedure is the prompt administration of


first-aid to accident victims and to those who become ill until medical aid can be fully
given by a medical doctor. This is paramount in order to ensure that the life of a casualty
is to be sustained and recovery is to be promoted. It is the Contractor’s policy that our
Site Manager would have attended a course in first aid, and have some knowledge of
what to do in an emergency situation.

1.2 Action to be taken


In the event of an accident, the Site Manager as the site first Aider to do the following:

i. Call -------------------------------Hospital, at-------------------------------------------,


and request for an ambulance to be sent immediately.

ii. Remain calm and assess what has happened

iii. Talk and listen to the casualty, and apart from instilling confidence and
reassurance, try to diagnose what injuries have been sustained.

iv. Give immediate treatment after making sure there are no further dangers
(collapsing structure, fire etc.).

v. In case there are immediate dangers, deal with them to reduce or eliminate further
accident. This may require the removal or deliberate demolition of the permanent
structure.

vi. Move the casualty to the safest minimum distance before giving first aid
treatment.

vii. All efforts must be made to ensure that the casualty is breathing properly and
thereafter place him in recovery position, if he is unconscious or likely to pass
out.

viii. Arrest bleeding

ix. Transfer the casualty to Hospital for further treatment.

x. Make adequate and accurate record of the accident.

xi. Report the incident on the same day to the Managing Director of that Contractor
Company
2.0 FIRST AID

2.1 Purpose
Provisions will be made for First Aid on the site in order to address the under listed
situations:

 Treatment of minor injuries which would otherwise receive no further treatment


or which do not need treatment by a medical doctor.
 Treatment for the purpose of preserving life and minimizing the consequences of
injury or illness until help is obtained from a medical doctor.

2.2 First Aiders


It is proposed that the Site Manager will undergo first Aid Training to enable him
perform the role of First Aider on the site.

It is also proposed that there will be a standing arrangement with the --------------------
Hospital, ------------------------- to receive and treat site personnel that may require the
help of a doctor, as a result of site operations.

2.3 Record of Treatment


All First Aid treatment will be recorded in the Special Accident Book to be made
available on site.

2.4 First Aid Boxes and Materials


The First Aid materials shown in the Table below are to be made available on site. Each
item used from the First Aid Box will be replaced promptly.

3.0. HEALTH AND SAFETY POLICY


3.1. General Policies
Contractor seeks to provide, safe working conditions that are reasonably practicable, by
adopting health and safety measures with emphasis on workforce involvement and full
commitment by the Managing Director.

3.2. Safety Arrangements


3.2.1 Contractor will make all who are at risk aware of the health hazards, the reason for
control in working practices and the part they, as individuals, have to play in maintaining
a safe and healthy work environment.
3.2.2 The Site Manager {SM}, will identify main hazards on the project and, additional rules
and regulations will be observed.
3.2.3 Procedures will be laid down for accident reporting and the results of accident analysis
will be made available to all site personnel.
The safety policy will be brought to the notice of all site personnel by way of displaying
it on a secured notice board on site and in the workshop
4.0. THE OBJECTIVES OF THE PLAN

This Project Health and Safety Plan has been developed specifically for the proposed
residential development at no 71 Herbert Macaulay Street, Ebute- Metta Lagos Mainland
Local Government Area Lagos State. The building is a two-storey building comprising of a
car pack at the ground floor level and one flat each on the upper floors that is the first and the
second floor.
The objectives include;
i) To secure the health, safety and welfare of persons who will work or visit the site;

ii) To protect persons other than persons in (i) above against risk to health and safety
arising out of, or in connection with the activities of persons working on the
project;

5.0 ASSESSMENT OF HAZARDS AND RISKS

5.1 After a careful study of the production information, in-depth site investigation and
analysis of the construction methodology, the project is judged to have a low content of
health and safety risks. The risks are as a result of the following:

1. Excavate: Possible collapsed of the sides of trenches, septic tank and soak away, if there
is no earthwork support falling into the trench.
2. Erection of form work: Fall from high level during erection of form work.
3. Concreting: Fall during casting of concrete.
4. Roofing: Falling from roof level during the construction of roof wall structure and roof
covering.
5. Nail: If site is not tidy and used planks well kept.
6. Site Personnel: The large number of workers to be engaged on the site. It is anticipated
that the site population at peak period will be about 60 people.
7. Falling Objects: Objects that could fall from high level in the buildings.

5.2 POTENTIAL HEALTH AND SAFETY OPERATIONS AND MATERIALS


The under listed operations and materials have been identified as those that must be
properly addressed in this Project Health and Safety Plan:

1. Excavation.
2. Off – loading vertical movement of materials and components.
3. Erection, adaptation and dismantling of scaffold.
4. Mechanical and Electrical Service holes in floors.
5. Mechanical and Electrical Service ducts.
6. Fall from scaffolding.
7. Positioning of Heavy Generator and other Plants.
8. Wood containing nail.

6.1 DUTIES OF CONTRACTORS

As a result of the provisions of common law, Decree No. 16 (The factories law 1987) and
Decree No. 17 (The workmen’s compensation law 1987) which places a general duty on
every employer to ensure, so far as is reasonably practicable, the health, safety and
welfare at work of all his employees, on this project the contractor will:

* Provide and maintain plant and systems of work that are safe and without risks to
health.

* Provide adequate welfare facilities and arrange for the welfare of site personnel
on the project.

* Provide a safe working environment that is without risk to health.

* Provide a safe place of work, with safe access and egress.

* Ensure safety, and absence of risks in the use, handling, storage and transportation
of materials and components.

* Provide adequate site management, supervision and inspection, as is necessary, to


ensure the health and safety of our employees.

* Provide adequate instruction as is necessary, to ensure the health and safety of our
employees.

* Provide any necessary information to ensure the health and safety of our
employees.

* Ensure, so far as is reasonably practicable, that our activities do not endanger


persons not in our employment who may be affected by operations under our
control e.g., subcontractors or designers and the public.

6.2. DUTIES OF OUR SITE PERSONNEL (CONTRACTOR’S EMPLOYEES AND


SUBCONTRACTOR’S PERSONNEL)

In line with the Contractor Health and Safety policy, the following duties are placed
on our employees.
i. To cooperate with the Contractor, as far as may be necessary, to enable us carry
out our legal and moral duties in health and safety matters.

ii. Not to intentionally or recklessly interfere with anything provided in the interests
of health, safety and/or welfare.

iii. To exercise reasonable care for the health and safety of himself and others who
may be affected by his acts of commission or omission at work.

iv. To report all accident to the SM immediately.

7.0 HEALTH AND SAFETY COMMITTEE

The Site Manager will have as part of his brief for managing the production of the project
the setting up of a Health and Safety Committee.

The following are to be member of the committee:

-Construction Manager {Main Building} -Member
-Construction Manager {Service Building} -Member
-Electrical Coordinator -Member
-Scaffold Supervisor -Member
-1 Bricklayer -Member
-1 Carpenter -Member
-1 Steel Fixer -Member
-Electrician (Temporary lighting) -Member
-Representatives of subcontractors -Members

While it is the responsibility of the Site Manager to ensure the health and safety of all site
personnel, the Committee will assist him in the implementation of the Health and Safety
Plan on the project. The Committee will meet at least once in a month or as
circumstances may warrant.

One of the duties of the committee is to investigate incidences of accidents on site and
carry out the analysis with a view to recommending practical solutions to stop future
occurrence.

Subject to the approval of the managing director, the Committee can amend (by way of
introducing stricter control measures) any aspect of this plan.

7.1. HEALTH AND SAFETY BRIEFING

Every person who will work on the site will be issued with identity card. No one will be
allowed to work on the site except he or she has been issued with an identity card. Before
the issuance of an identity card, a person will be thoroughly briefed about the health and
safety requirements on the site. After the Health and Safety briefing session, the person
will sign the Health and Safety register. Signing the Health and Safety register will be a
condition precedent to issuance of identity card.

The Health and Safety briefing is to be conducted by one of the Construction Managers.
The briefing, among other things, will include showing the person the Health and Safety
provisions available on site, what to do and what not to do on site, precautions that need
to be taken on site and his duties regarding health and safety as a worker on the site.

All employees will be made aware of the location of first aid facilities and the
arrangement for obtaining treatment.

8.0. SITE ACCOMMODATION AND WELFARE FACILITIES

8.1. Shelter
As part of the site accommodation, provisions will be made for shelter for site the
operatives. The shelter will be provided with hooks for hanging cloths and benches for
seating. It will also be provided as a changing room, resting place during break,
protection from rain and for storing work clothing.

8.2. Drinking Water


Drinking water will be provided from the overhead water storage tanks available on site.

8.3. Eating Place


Although provision is not made for the preparation of food on site, a 3m x2.4m shelter
with benches will be provided on site as eating-place. This will enable site operatives to
bring food to site and eat in a conducive environment.

8.4. Washing Facilities


Near the sanitary conveniences, there will be a covered area set aside for operatives to
wash after each day’s work. There will be sufficient water on site for this purpose.

8.5. Sanitary Conveniences


A pit latrine with divided into sections will be provided on site. One of the sections is
actually reserved for the females that may work on or visit the site.

8.7. Maintenance of Welfare Facilities


Facilities provided on site for the welfare of site personnel will be maintained and have
safe access. To achieve this, one unskilled operative will be employed for cleaning the
facilities.

9.0 ACCIDENT PREVENTION MEASURES

9.1 General
The accident trend in the construction industry can only be influenced positively by
providing adequate training and management in order to control the workers, the
plant/equipment and the working environment.

To this effect, the following measures are to be taken on the site.

9.2 Workers
 Each worker will be properly informed of the activities he is expected to perform
and be made aware of all the inherent hazards.
 Only competent workers will be engaged on the project.
 Safe system of work with the use of work instruction sheets will be adopted for all
operations.
 Workers will be made to use the protective clothing and equipment provided.
 They will be made aware of accident and emergency procedures through the
Health and Safety briefing session.
 They will be made aware of cost of accident.

9.3 The Plant and equipment


The SM will make sure that the following procedures and practices are observed:

 Regular inspections by trained and competent persons are carried out.


 No defective equipment is used.
 Defect on tools, plant and equipment are quickly and properly rectified.
 Records and reports are properly and accurately maintained.
 Proper access and egress are provided.
 All moving parts are adequately guarded and protected at all times.
 Hand tools are inspected and maintained.
 Proper handling, lifting, slinging of equipment.
 Equipment and machines are properly secured when in use and when parked.

9.4 The Work Environment


This will apply to all areas of the site including the stores, offices and welfare facilities.
Resources are to be made available and regular checks are to be carried out in order to
ensure that:

 Floors are clean, not slippery and free from debris. This will be achieved by the
employment of unskilled operatives who will be specifically responsible for
tidying up the entire site.
 Stairs, gangways and loading bays are adequately guarded and maintained by the
use of either wooden or steel guardrails. The workplace is adequately lit, in
particular at access points and openings and whenever lifting operations take
place.
 Edges, openings and holes are adequately protected by covering them with wood
firmly secured to the slab by using concrete nails.
 Display of standard warning signs where required or where hazards exist.
 Materials and components are stacked or stored correctly.
 Projecting objects or obstacles are protected and guarded.

10.0 PROTECTIVE CLOTHING AND EQUIPMENT

Provision of necessary protective clothing and equipment by CONTRACTOR for the


project cannot be over emphasized; so also, is the subsequent use of them by all site
workers. For the safety of site workers, the following protective clothing and equipment
will be provided on site:
 Safety Helmets
 Safety boots
 Industrial Gloves
 Dust mask

The site is to be a Hard Hat site.

11.0 PERMIT – TO – WORK

A formal permit – to – work system will be strictly employed on the project for the
following:

 Basement excavation
 Access to scaffolding
 Use of Electrical Operated Hand Tools
 Generator House
 Electrical Switch rooms

The Site Manager will sign the permit –to – work.


The system will enable those engaged on the work to see exactly what has been done,
what is expected of them, and what precautions they must take.

12.0 ACCESS AND EGRESS FROM WORK PLACE


Attempts to gain access or egress from work place through unauthorized means will be
totally discouraged. To achieve safe access and egress from work place, the following
actions will be taken:

 The permanent stairs in the building will be constructed as the building grows.
 The stairs will be provided with guardrails.
 Independent scaffolding will be erected as perimeter block work and cladding
proceeds.
 Wooden ladders of various lengths will be provided for access onto scaffolds.
 Wooden ladders will also be provided for getting into and climbing out of
basement excavation.

13.0 UNDERGROUND OBSTRUCTIONS AND BURIED SERVICES

13.1 In view of the fact that buried services are largely, out of sight and out of mind until the
time come for someone to dig a hole or start excavating for trenches for substructure, the
need for its inclusion in this plan cannot be over-emphasized.

The most obvious examples of buried services are those used to carry water, electricity,
telecommunications and gas. The same is true of drains and sewers.

13.2 Locating and Identifying Buried Services


Before any digging takes place, checks will be carried out in conjunction with public
utility officials. The checks will be carried out in order to identify the existence or
otherwise of services in the proposed work area. Any services found would be clearly
marked on the site layout drawing and the route physically identified on ground with red
paint.

14.0. ERECTION AND INSPECTION OF SCAFFOLDS

14.1 General
All scaffolds and working platforms are to be properly erected to provide adequate
working space. No scaffold will be erected, modified or dismantled except under the
supervision of Scaffold Supervisors, Mr.……………………… and Mr.………………….

All scaffold materials are to be inspected by one of them.

Scaffold and Platform Towers will be provided on site for all work which cannot safely,
be done from the ground or part of the building. While the scaffold to be used on the
project will be bamboo with timber boards, the platform tower will be purpose made
wooden towers. Scaffold will be properly maintained and inspected frequently to check
all serviceable parts and secured to prevent accidental displacement. No scaffold shall be
left partly erected or dismantled unless adequate notices are displayed and access
blocked.

15.0 FIRE PREVENTION AND PROTECTION PROCEDURE

To prevent and deal with occurrence of fire on site and to protect the works, the
following actions are to be taken:
i. No inflammable materials are to be stored on site within the building.

ii. Photoelectric smoke detector warning alarms will be strategically positioned and
protected on site and on each floor of the building.

iii. All the stairs will be made clear of debris and completely free of all obstruction so
that they can be used as fire escape routes.

iv. As part of the Health and Safety briefing session, site personnel will be informed
of evacuation plan.

v. The Site Manager will have the telephone number of the ………… ………….Fire
Station for quick contact:

vi Access for fire engine will be provided at the entrance to the site.

vii. Two fire extinguishers will be made available on site. They will be inspected and
tested periodically by experts from outside Contractor’s company

vii. There will be periodic fire drills throughout the duration of the project.

16.0 HEALTH AND SAFETY RECORDS

The Site Manager will keep records of health and safety matters on the project as it
relates to the following:

16.1 Health and Safety Administration


i) Health and Safety Register.
ii) Health and Safety Briefing sessions.

16.2 Accident or Dangerous event


i. Date and time of accident or dangerous occurrence

ii. Particulars of person(s) affected


 Full name
 Trade
 Nature of injury or condition

iii. Place where the accident or dangerous occurrence happened

iv. A brief description of the circumstances


v. Action(s) taken to administer treatment/remove dangerous occurrence.

16.3 The SM will also keep records in any case where diseases are reported, details to include:

i. Particulars of the person affected,


ii. Date of diagnosis of the disease,
iii. Trade of the person affected,
iv. Name and nature of the disease.
v. Treatment / action taken.

These records are to be kept at least three years after the practical completion of the
project.

17.0 SAFETY RULES AND PROCEDURES


1. No employee is expected to take a job until adequate training has been taken
2. All employee shall be trained on every potential hazard that they could be exposed to and how
to protect themselves.
3. No employee is allowed to work under conditions which are unsanitary, dangerous or
hazardous to health.
4. Only qualified trained personnel are permitted to operate machinery or equipment.
5. All injuries must be reported to the supervisor’s/foreman.
6. Manufacturer’s specifications/limitation/instructions shall be followed
7. Particular attention will be given to new employees and to employees moving to a new job or
doing non-routine tasks.
8. All safety posters shall be posted
9. Emergency numbers shall be posted and reviewed with employees.
10. Each employee in an excavation/trench shall be protected from cave-ins by an adequate
preventive system.
11. Employees working in areas where there is a possible danger of head injury, excessive noise
exposure, or potential eye and face injury shall be protected by personal protective equipment
(PPE).
12. All hand and power tools and similar equipment, whether furnished by the employer or the
employee, shall be maintained in a safe condition.
13. All materials stored in tiers shall be stacked, racked, blocked, interlocked, or otherwise
secured to prevent sliding, falling or collapse.
14. The employee will ensure that the electrical equipment is free from recognized hazards that
are likely to cause death or serious physical harm to employees.
15. All scaffolding shall be erected in accordance with the standard guardrails for fall protection
and ladder for safe access shall be used.

1.0 EMERGENCY ACTION PLAN

1.1 Emergency escape procedure

All employees will immediately leave the building through the closest practical exit. Meet up at
the foreman’s truck.
Critical plan operation – Shut off the generator on your way out if possible, otherwise evacuate
the building
1.2 Accounting for employees

Foremen/Supervisor’s is to account for all employees after emergency evacuation has been
completed assign duties as necessary.
Means of reporting fire and other emergencies – dial 911 on the cell phone, report the location of
the emergency and provide direction to responders.

duties under plan.
Alarm system/evacuation – The company establish the cell: fire, fire, fire: by any employee, as
the signal to immediately evacuate the building/facility for: fire and other emergencies.
Training – before implementing the emergency action plan, a sufficient number of persons to
assist in the safe and orderly emergency evacuation of employees will be designated and trained.
The plan will be reviewed to each employee covered by the plan at the following time
1.Initially when the plan is developed or upon initial assignment.
2.Whenever the employee’s responsibilities or designated actions under the plan change.
3.Whenever the plan is changed
The plan will be kept at the worksite and made available for employee review.
