CALL FOR SUBMISSION OF DOCUMENTS FOR ASSESSMENT OF


TEACHER IN-CHARGE (TIC)

TO: School Principals/Head Teachers/Teachers In-Charge

1. This office announces the submission of documents for assessment of three


vacant positions for Teachers In-Charge on September 8, 2021, from 7:00 am
to 5:00 pm at the District Office.

2. All School Heads to inform ALL INTERESTED AND QUALIFIED


TEACHERS to be ready for the scheduled assessment on September 10, 2021
(Friday), the venue to be announced later.

3. The qualification standards for the position are as follows:


a. Permanent teacher
b. At least 5 years’ experience as Teacher 3.
c. Bachelor’s degree for teacher or Bachelor’s Degree with 18 units
professional units in Education.
d. With 40 hours of relevant training.

4. Interested applicants should meet the qualification standards prescribed in


DepEd Order No. 42 s. 2007 and the basic qualification of the position per
qualification standards of the Civil Service Commission.

5. Interested applicants shall personally hand-in or send through their school


heads their application portfolio on the scheduled day of submission, arranged
and properly labelled in three (3) copies together with the following
documents:

A. Letter of Intent (duly signed);


B. Personal Data Sheet CSC Form No. 212, Revised 2017) duly signed with
recent passport size picture;
C. Performance Rating for the last three rating periods;

 SY 2019-2020

 Memorandum/Advisory regarding the conduct of the Search
 Approved Criteria for Judging
 And other relevant documents regarding the search.
ii. Innovation supported by the approval through channels of the
Innovative Project, Abstract of the Project and Workplan
conducted/implemented, Completion Report/ Impact of the
Workplan and other MOVs;
iii. Research and Development Projects supported by the approval of
the request, thru channels, to conduct the research, The Research
Proposal, Findings and Recommendations and other MOVs;
iv. Copy/ies of published articles which must be educational articles
and not news, published in newspapers/magazines of wide/
national circulation; for published books, it must be with ISBN;
v. Certificate of Recognition as Consultant/Resource Speaker in
Training/Seminars conducted in any of the following levels:
school/district/division/province/regional/national/international,
supported by the following documents:
 Invitation
 Travel Order
 Training Matrix
 Memorandum regarding the activity
F. Transcript of Records (TOR), authenticated and certified in the College
Level/Post Graduate Studies-Master’s Degree/Doctoral Degree.
G. Certificate/s of Participation to Trainings/Seminars attended, with the
following supporting Documents:
 Approved Travel Order

i. Three (3) Certificates of Participation to
Trainings/Seminars conducted for at least 3 days each
training for trainings conducted in the
district/division/province and regional levels;
ii. One (1) certificate of participation conducted for at least
3 days for trainings conducted in the national or
international level.
H. Eligibility and License duly certified and authenticated by the issuing
agency; and
I. Omnibus Certification of authenticity and veracity signed by the
applicant
