We thank you for choosing us as your preferred Health Insurance partner and for continued patronage.

We would like to inform you that your existing group master policy 0239326260 with Axis Bank is being discontinued as on 01-07-2024,
however your coverage under the current policy certificate continues till your certificate expiry date i.e., 01-07-2024. In order to ensure
continuity of benefits under your current policy certificate after certificate expiry date, you can migrate to our individual health
insurance product “Tata AIG MediCare” UIN No: TATHLIP23118V032223.

Key details of the revised benefits and premium applicable upon migration are enclosed for your reference. We request you to migrate
your certificate before the expiry date to enjoy continuity of your insurance coverage. Upon timely migration to TATA AIG Medicare, all
your continuity benefits including waiting periods would be transferred to this product. Upon migration to Tata AIG MediCare, you will
receive a revised policy number.

Please note that any auto debit instruction given by you in the past for debit of renewal premium on the aforementioned policy stands
cancelled with immediate effect as the product has changed.
