New Life đề nghị Quý khách lưu ý, ngay sau khi New Life nghiệm thu và bàn giao sử dụng theo yêu cầu lắp đặt mới dịch
vụ (1) cho Quý khách. Nếu không còn nhu cầu với Hợp Đồng Dịch Vụ Internet Hiện Tại(2), Quý khách vui lòng liên hệ
ngay điểm giao dịch của New Life để thực hiện thủ tục tạm ngưng hoặc thanh lý Hợp Đồng Dịch Vụ Internet Hiện Tại (2)/
New Life recommends you to be aware, as soon as New Life has accepted and handed over the service at the request for
new installation of service(1) for you, if there is no longer demand with the Internet Service Contract(2), please contact New
Life's transaction office to implement procedures for suspending or liquidating the Current Internet Service Contract(2).
Trường hợp Quý khách không liên hệ trực tiếp New Life để thực hiện tạm ngưng hoặc thanh lý theo quy định thì New
Life/ Nhà mạng vẫn duy trì và tính cước đối với Hợp Đồng Dịch Vụ Internet Hiện Tại (2). Mọi sự chậm trể của khách
hàng trong việc thực hiện các thủ tục cần thiết tại Thông báo này, Quý khách sẽ hoàn toàn chịu trách nhiệm/ In case you
do not contact New Life directly for carrying out the procedure of suspension or liquidation as prescribed, New
Life/Provider still maintains and charges for the Current Internet Service Contract (2). Any delay of customers in
implementing necessary procedures in this Notice, you will be solely responsible.
Vì lý do sau /Because of the following reasons:
- Yêu cầu lắp đặt mới dịch vụ (1) của Quý khách và Hợp Đồng Dịch Vụ Internet Hiện Tại(2) mà Quý khách đã giao kết
với New Life trước đây là hai Hợp đồng khác nhau. Quyền của khách hàng là có thể sử dụng đồng thời nhiều đường
dây dịch vụ Internet khác nhau tại cùng một địa chỉ, nên việc yêu cầu lắp đặt mới dịch vụ(1) của Quý khách không
được mặc định hiểu rằng New Life sẽ tự động hủy dịch vụ mà Quý khách đang đứng tên Hợp Đồng Dịch Vụ Internet
Hiện Tại(2)./ Your request for new installation of service(1) and the Current Internet Service Contract(2) that you have
previously entered into with New Life before are two different contracts. It is the customer's right to be able to use
many different Internet service lines simultaneously at the same address, so your request for new installation of
service(1) is not understood by default that New Life will automatically cancel the service for which you are under the
name on the Current Internet Service Contract(2).
- Theo quy định của pháp luật và Hợp Đồng Dịch Vụ Internet Hiện Tại(2), trường hợp không còn nhu cầu sử dụng dịch
vụ tại địa chỉ đăng ký theo Hợp đồng đã giao kết, khách hàng có trách nhiệm thông báo đến bên cung cấp dịch vụ để
yêu cầu chấm dứt Hợp đồng hoặc chuyển đổi quyền sử dụng. Việc yêu cầu chuyển quyền sử dụng, tạm ngưng/tái lập
dịch vụ theo yêu cầu hoặc thanh lý/chấm dứt sử dụng đều phải được xác lập bằng văn bản (“Phiếu yêu cầu”) giữa thuê
bao và bên cung cấp dịch vụ. Trên cơ sở đó, bên cung cấp dịch vụ sẽ thực hiện thủ tục thanh lý với khách hàng./
According to the provisions of the Law and the Current Internet Service Contract (2), in case there is no need to use the
service at the registered address under the signed contract, the customer is responsible for notifying the service
provider to request the termination of the Contract or transfer of the use rights. The request to transfer the use rights,
suspend/re-store the service as request or liquidate/terminate the use must be made in writing ("Request form")
between the subscriber and the service provider. On that basis, the service provider will carry out the liquidation
procedure with the customer.
Trường hợp không còn nhu cầu với Hợp Đồng Dịch Vụ Internet Hiện Tại(2), Quý khách vui lòng điền biểu mẫu (tại mặt
sau) của Thông báo này. Nếu vẫn có nhu cầu sử dụng dịch vụ hiện tại, Quý khách vui lòng bỏ qua biểu mẫu đính kèm
Thông báo này./ In case there is no demand for the Current Internet Service Contract (2), please fill out the form (at the
back) of this Notice. If you still want to use the current service, please ignore the form attached to this Notice.
Thông báo này được lập thành một bản gốc và một bản sao, New Life giữ một bản gốc và Quý khách giữ một bản sao./
This notice is made into one original and one copy, New Life keeps one original and you keep one copy.

of service with New Life) agree that before signing the Minutes of acceptance and handing over the service at the request
for new installation of service(1), will return the device and temporarily pay charges for the Current Internet Service

Temporary payment charges amount to be calculated until the end of the date at the time of acceptance and handing
over of service at the request for new installation of service(1) according to the receipts issued by New Life.

Sau khi tôi hoàn trả thiết bị và tạm thanh toán số tiền nêu trên, đề nghị New Life thực hiện tạm ngưng dịch vụ của Hợp
Đồng Dịch Vụ Internet Hiện Tại(2). Đến khi có thông báo cước chính thức, tôi sẽ liên hệ trực tiếp với New Life để thực
hiện thủ tục thanh lý hợp đồng và quyết toán nợ đọng.
After I return the device and temporarily pay the above amount, request New Life to suspend the service of the Current

procedures for contract liquidation and debt settlement.
Sau 30 ngày kể từ ngày hết thời hạn được quyền tạm ngưng (nêu tại Hợp Đồng Dịch Vụ Internet Hiện Tại (2)) hoặc 60
ngày, kể từ ngày tạm ngưng (thuê bao của Viettel), nếu tôi không khôi phục sử dụng hoặc thực hiện thủ tục thanh lý Hợp
đồng với New Life thì New Life được quyền đơn phương chấm dứt Hợp Đồng Dịch Vụ Internet Hiện Tại (2), mà không
phải hoàn trả phí lắp đặt ban đầu và các loại phí sau lắp đặt cho Quý khách.
After 30 days from the date of expiration of the right to suspend (stated in the Current Internet Service Contract (2)) or 60
days from the date of suspension (Viettel's subscriber), if I do not re-store the service or perform the liquidation of the
Contract with New Life, New Life is entitled to unilaterally terminate the Current Internet Service Contract(2), without

Trường hợp tôi thay đổi ý kiến hoặc người đại diện tôi ký xác nhận Biên bản nghiệm thu và bàn giao dịch vụ mới (1) không
đồng ý hoàn trả thiết bị và tạm thanh toán số tiền nêu trên. Đề nghị New Life vẫn duy trì và tính cước đối với Dịch Vụ
Internet Hiện Tại (2). Tôi sẽ chịu trách nhiệm thanh toán mọi khoản cước phát sinh và bồi thường (nếu có).
In case I change my mind or my representative who signs for confirmation on the minutes of acceptance and handing over
of service at the request for new installation of service(1) does not agree to return the device and temporarily pay the above
amount, I suggest New Life maintains and charges for the Current Internet Service Contract(2. I will be responsible for
paying all arising charges and compensating (if any).
Tôi đã đọc, hiểu rõ về nội dung khuyến cáo của Thông báo này./ I have read and understood the recommended content
