1) When can we define that employee is entering in a CSE?

• “Confined space Entry” (CSE) begins as soon as a person’s whole body or


upper body or head crosses the confined space’.

2) Give 3 reasons why watchman has to be present at CSE entrance:

• To control the entry by ensuring that only trained personal with all PPE
required is entering the CSE once the atmosphere is safe;
• To ensure communication with the entrance and call them to exit if required;
• To actively monitor the atmosphere condition continuously before entering,
during the task and launch evacuation if one condition is not respected;

3) Give 3 responsibles making sure that CSE activity is safe for the task:
• Equipment preparer: ensuring that CSE preparation is done as required
flashing, draining, purging and positive isolation of the equipment.
• Initial Gas tester: Verify CSE Air quality is safe for the task O2 between 19.5%
and 23%, LEL less than 5%.
• Permit coordinator: Ensure equipment is well prepared, risk evaluation is
done, all documents available and confirm field status of the equipment before
issuing permit.
• Permit acceptor: Ensure that employee is trained; JSEA shared and
understood by the team, control measure applied during the task and supervise
the team to follow safety instruction and to wear PPE as required.
• Permit user: Follow instruction and apply PTW and JSEA control measure and
conduct Take 5 before working.
• Watchman: Actively keep two way communications with the CSE entrants,
control the access and install signage before leaving the entrance.
4) Reminder: When does the CSE entrant need to evacuate confined
space?
• Personal or multi-gas detector triggered.
• Local alarm or general siren activated or evacuation requested by the
watchman.
• In case of any abnormal situation which may impact the CSE activity.
• When the SABA system pressure is less than 500 PSI.
• SKA-PAK pressure is less than 3000 PSI.
