I, FRANCISCO ROLA GAMATERO, Filipino, of legal age, married, a

retired government employee and a resident of Calanan, Tabuk City, Kalinga,
Province, after having been duly sworn in accordance with law, hereby depose
and say:

1. That I am a holder of Checking Account duly issued by the Land Bank of

the Philippine with Account Name FRANCISCO R. GAMATERO and
Account Number 1052004841;

2. That the abovementioned account had a total deposit of One Hundred

Sixty-One Thousand and Thirty-Eight (₱168,038.00) Philippine Currency;

3. That I would like to transfer the said amount in my Land Bank Savings
Deposit FRANCISCO R. GAMATERO with Account Number 1051-0784-
85;

4. That I am executing this affidavit to attest to the truth of the foregoing

facts and for whatever legal purposes it may serve.
