I understand and confirm the following information about my Home Credit Protect - Borrower Protection:
1. I received Confirmation of Cover of Insurance No. 4403597049 issued by SUN LIFE GREPA FINANCIAL, INC. (“SLGFI”) thru HC Consumer Finance Philippines, Inc.
(“HC”). These insurance policies provide benefits for hospitalization or death of any cause, as well as Telemedicine services defined by the terms and conditions of the
Group Master Policy issued separately by SLGFI.
In no event will SLGFI pay more than one (1) month’s worth of installment if Debtor is confined more than once during a monthly installment period. The total monthly
installments payable to the Creditor shall not be cumulative.
2. I understand that monthly installments for my loan, including the insurance fee, must always be paid on time to avoid possible cancellation of my insurance coverage.
3. I can cancel my insurance anytime within the loan term and premiums paid before cancellation are non-refundable. Cancellation of insurance will exclude premium
payments on my next loan payments. Should I call HC within 3 days before my due date, cancellation will take effect on the next due date.
4. I can designate my beneficiary/ies by sending an email to SLGFIAMCustomerService@sunlifegrepa.com. Should I fail to designate my preferred beneficiary/ies, the
insurance proceeds shall be payable to the first surviving class of the following order of classes of beneficiaries deemed named by the Member:
1. Widow/Widower
2. Surviving legitimate and illegitimate children;
3. Surviving parents;
4. Surviving brothers and sisters of the full blood;
5. Surviving brothers and sisters of the half-blood; or
6. Executors, administrators or assigns.
If two or more beneficiaries belong to the same class entitled to the insurance proceeds, such beneficiaries shall share equally. Any minor's share shall be paid in the
manner provided by law.

This document describes in general the insurance protection under the Policy as required under Sec. 234 of the Insurance code. The full details are in the
Policy and in the event of discrepancy or dispute, the provisions of the Creditor Group Life Policy shall prevail.
By availing of an insurance coverage under this Policy, the Debtor agrees that Sun Life Grepa Financial shall process his personal data to: (i) evaluate his
application and administer his account; (ii) process claims and enforce/fulfill contractual rights/obligations; (iii) improve the provision of products and services
(including improvement in systems and business processes, data analytics, automated processing, etc.); (iv) comply with legal obligations, as well as laws
and regulations (domestic or foreign); and (v) manage risks and pursue its legitimate interests, including verification and obtaining additional personal data
from third party sources. Sun Life Grepa Financial may disclose his personal data to its affiliates, service providers, and other third parties for processing
consistent with the foregoing purposes, who shall be bound by contractual or other reasonable means to protect the Debtor’s personal data.
The Debtor’s personal data shall be retained for the duration of his coverage under his plan or existence of his account(s) and/or upon the later of the
expiration of the retention limit set by Sun Life Grepa Financial standards, laws and regulations, counted from account closure. The Debtor certifies that he

The amount of insurance is subject to the terms and conditions set forth in the Group Personal Accident Policy and any rider attached to it, issued to the Policyholder. In the event
of claim, Sun Life Grepa Financial shall pay the Member, if living, otherwise, his beneficiary or beneficiaries the benefit stated in the Policy.
a. sustains loss from accidental bodily injury as evidenced by an external and visible contusion or wound except in the case of drowning or internal injuries shown by medical
examination or autopsy, and resulting directly and independently of all other causes; and
b. receives treatment commencing within thirty (30) days from the date of such accident from a qualified Physician or while confined in a licensed Hospital; and
c. incurs expenses for medical, hospital or surgical treatment of such bodily injury only;
Sun Life Grepa Financial shall reimburse the actual, necessary, reasonable and customary expenses which [a Member/an Employee/or Dependent] may have incurred for no
more than fifty-two (52) weeks after the injury up to the maximum amounts specified in the Amount of Insurance, subject to the Exclusions and other provisions of the Policy and
this Medical Reimbursement Benefit.
Hospital shall mean any private or government institution legally constituted locally, and operated hospital or clinic duly registered with the Bureau of Medical Services,
Department of Health, and located within the Philippines. It shall have organized facilities providing medical, diagnostic and surgical facilities for the care and treatment of the sick
and injured person on an inpatient basis under the supervision of a physician with 24-hours nursing service by registered graduate nurses and which is not, other than incidentally,
a place of rest for the aged, drug addicts, alcoholics or convalescent home.
Physician shall mean a legally licensed physician or surgeon duly registered and practicing within the scope of his profession. A Physician cannot be the Member; or the brother,
sister, parent, spouse, or child of the Member, unless approved by Sun Life Grepa Financial.
Coordination of Benefits
This Policy shall not cover hospital confinement, services, supplies, treatment or any other medical care which are furnished or for which benefits are payable under any other in
force policy or Plan, or under any extension of benefits provisions of any other such policy or Plan which has been canceled; provided that, if the benefits payable under such other
policy or Plan are less than the total expenses incurred by the[Member/Employee/or Dependent], Sun Life Grepa Financial shall reimburse an amount up to the benefits provided
under this Policy. In no event, however, shall the total payments from this Policy and such other policy or Plan exceed the actual incurred expenses of the insured Member.

HOME CREDIT TELEMEDICINE PROGRAM
The pandemic has changed everyone’s lives across all walks of life regardless of status, profession, business etc. In its continued desire to provide
value added products and services to its borrowers, Home Credit Philippines (HCPH), in partnership with Grepa Medical and Diagnostic Center
(GMDC), a division of Sun Life Grepa Financial, Inc. (SLGFI), is introducing the Home Credit Telemedicine Program (HCTP), a new feature of the
Borrower Protection Plan.
The HCTP will provide Borrowers an online access to a Physician / Medical Doctor (MD) without the hassle of visiting the hospital or clinics.
OPERATIONAL GUIDELINES
A. PATIENT SCHEDULING
A. HCPH Borrower (HCTP Patient) calls hotline or email for scheduling or queries regarding health conditions.

B. Calls and emails will be received by a HCTP Coordinator for proper processing.
C. HCTP Coordinator will ask HCTP Patient for the chief complaint (if COVID related or other Medical Assistance).
D. HCTP Coordinator to advise the HCTP Patient that Zoom App platform will be used for the consultation.
E. HCTP Coordinator will request the following information through email or call
• Full Name
• Birthdate
• Gender
• Home address
• Birthdate
• Mobile No. or Telephone No.
• Email address to where teleconsult will be linked
• Loan account number (for verification if patient is a HCPH member)
F. HTCP Coordinator acknowledges the receipt of the basic information listed above. Patient to be instructed to download ZOOM app
and their email will be linked accordingly.

Service, (b) may report to law enforcement authorities any actions that may be illegal, and any report it receives of such conduct. When
legally required or at SLGFI's discretion, SLGFI will cooperate with law enforcement agencies in any investigation of alleged illegal
activity on this Service.
