RE: PROPOSAL FOR TAX AND ADVISORY SERVICES

Dear Ms. Samarista:

This letter is to submit the proposal of our engagement as well as the nature and limitations of
the services that we will provide to BELLYSIMO HOMEMADE FOOD RETAILING.

The retainer’s fee is P5,600.00, INCLUSIVE of Value-Added Tax (VAT).

SCOPE OF SERVICES INCLUSIVE IN MONTHLY RETAINERS:

1. Preparation and electronic filing of the necessary BIR tax compliance reports such as
follows:
- Monthly, Quarterly and Annual Withholding Tax on Compensation, Expanded and
Final.
- Quarterly Alphalist of Payees (QAP)
- Monthly and Annual Withholding Tax Returns for Compensation (1601C and 1604C)
- Quarterly Income Tax Return (1701Q)
- Monthly and Quarterly VAT (BIR Form 2550M and 2550Q)
- Summary List of Sales and Purchases (VAT Relief)
- Annual Registration FEE (0605)

2. Providing financial advisory and tax consultation.

SCOPE OF SERVICES EXCLUSIVE TO MONTHLY RETAINERS:

BUREAU OF INTERNAL REVENUE

1. Renewal of Books of Accounts

2. Preparation of requirements (per BIR Examination and Audit / per LoA)
3. Preparation of ANNUAL financial statements and Income Tax Return

SECURITIES AND EXCHANGE COMMISSION

1. Preparation and filing of General Information Sheet

2. Amendment of Articles of Incorporation and by-laws

It is also understood that incidental expenses regarding the engagement shall be on the account and
shall be billed to you separately at cost.

In the event that either or both parties intend to terminate in part or the whole contract, a written letter
of notice shall be given at least one month before the effectivity date to give ample time for the turn-
over of documents.
