MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement executed and entered into and between:

APAYAO STATE COLLEGE an Institution of higher learning by Republic

Act No. 8563 located in Malama, Conner, Apayao and a Campus located
in San Isidro Sur, Luna, Apayao, represented by this College President
DR. JOHN N. CABANSAG, hereinafter referred to as the FIRST PARTY

and

STA. MARCELA CENTRAL SCHOOL, a public school located at

Marcela, Sta. Marcela, Apayao represented in this agreement by its
Principal, DR. MYRNA C. BALANAY, hereinafter referred to as the
“SECOND PARTY”.

WITNESSETH:

WHEREAS, the FIRST PARTY is mandated to perform its four-fold functions such as
Instruction, Research, Extension, and Production;

WHEREAS, the FIRST PARTY through its Research Development and Extension
Programs is tasked to plan, implement and evaluate its extension program such as
continuing education, literacy program, technical assistance, consultancy services,
environmental protection management, and community services;

WHEREAS, the FIRST PARTY through its Research Development and Extension Unit
shall implement extension services by sustaining partnership and linkaging with
government line agencies, non-government organizations and other stakeholders;

WHEREAS, the FIRST PARTY willing to conduct extension services through the
implementation of Project Science, Mathematics, Araling Panlipunan and Reading
and Tutorial (Project SMART for brevity)

WHEREAS, the SECOND PARTY is agree andf willing to participate in the

implementation of Project SMART which aims to develop its resources and establish
linkages to existing government and non-government organization, and individual
stakeholders in order to maximize resource generation and utilization;

WHEREAS, both parties recognize the importance of collaboration and partnership to

address the challenge of regional and national growth and development specially in
poverty alleviation and the improvement of quality of life;

NOW THEREFORE, for and in consideration of the above premises and mutual
covenant, both parties agree to the Memorandum of Agreement subject to the following
stipulations:
ARTICLE I – ROLES AND RESPONSIBILITIES OF THE PARTIES

Section 1. The FIRST PARTY shall:

1. Establish linkage with Sta. Marcela Central School;

2. Implement Extension Projects/Programs & Activities at Sta. Marcela Central

School;

3. The implementation of the Project SMART will last depending on the progress of
the struggling learners and the willingness of the Second Party to serve.

4. Provide technical and support services along the Apayao State College mandate
as covered by the School Development Plan;

5. Mobilize faculty members to undertake activities initiated by the school along

areas of:

a. Continuing Education and Literacy Program on the subjects like Science,

Mathematics, Araling Panlipunan and Reading and Tutorial to students
b. Research Mentoring of students

6. Monitor and evaluate the implementation of programs, projects, and activities;

and

7. Present to the school the monitoring and evaluation results.

Section 2. The SECOND PARTY shall:

1. Accommodate the free assistance/services provided by the college;

2. Willingly participate in the ASC Extension activities initiated in the school;

3. Recommend other activities as may be needed by the school; and

4. Provide policy to support Planning, Coordination, Implementation, Monitoring,

and Evaluation of programs, projects, and activities at Sta. Marcela Central
School.

ARTICLE II – SEPARABILITY CLAUSE

In an event that one or more provisions contained herein shall be held invalid, illegal or
unenforceable in any respect and for any reason, the remaining provisions shall remain
valid, legal, and enforceable.

ARTICLE III – DOCUMENTS COMPRISING THIS AGREEMENT

All appendages hereto attached are hereby made an integral part of this Agreement by
reference, inconsistencies with any/all part, terms and conditions contained in this
Memorandum of Agreement.

ARTICLE IV – EFFECTIVITY OF THE AGREEMENT

The terms and conditions of this Agreement shall take effect upon the approval and
signing by both parties for a period of one (1) year. Provisions in this Agreement may be
modified, altered, or supplemented subject to the mutual agreement of both parties.

ACNOWLEDGEMENT

BEFORE ME, a Notary Public in _______________________, this

above-named persons, who have satisfactorily proven to me their identities through
their competent identification as indicated below their names, that they are the same
persons who executed and voluntarily signed the foregoing Memorandum of Agreement
which they acknowledged before me as their free and voluntary act and deed.

This instrument, consisting of three (3) pages, including the page in which this
Acknowledgement is written, has been signed on each and every page thereof by the
