AFFIDAVIT

I, MARIA TERESITA ZATA PERILLA, Filipino, of legal age, single and a resident
Phase 2, B2B, Lot 2, St. Scholastica’s Village, Palo, Leyte, having been duly sworn in
accordance with law, hereby depose and say:

1. That I am the owner of a portion of a parcel of land designated as Lot No. 3686
located at Brgy. Patong, Pastrana, Leyte, consisting of Ten Thousand (10,000) square
meters, having bought the same from the Heirs of Dominador T. Cabidog per
Extrajudicial Settlement of Estate with Absolute Sale of Real Property entered in the
Notarial registry of Atty. Ma. Rowena F. Goc-ong as Document No. 312, Page No.
064, Book No. XII , Series of 2021 dated August 21, 2021;

2. That the said parcel of land has some improvements erected thereon including a
house made of light materials with an electric connection from LEYECO III;

3. That upon verification the electric meter and electric connection is under the name of
Regine B. Canete with meter number 12685543 and account No. 9090070240.
Further inquiry had been made to locate the said Regine B. Canete but she is nowhere
to be found and she can no longer be contacted in any ways by any resident in Brgy.
Patong, Pastrana, Leyte;

4. That the requirements from LEYECO III for the transfer of ownership requires
consent of the prior owner but the same cannot be complied due to the reason as
stated above;

5. That I hereby request the LEYECO III to transfer the said electric connection unto my
name for the purpose of continuing the said electric connection upon payment of
applicable fees and charges;

6. I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever legal purpose this may serve.
