Administration of the Year-end Division Integration On Assessment Of Learning
Monitoring And Evaluation (DIAL ME) of DepEd Division Of Cebu Province who
is tasked to receive the test questions and administer, I hereby SOLEMNLY
SWEAR that I will strictly observe security measures to maintain the
confidentiality of said test questions and administer following guidelines,
