    Ossining High School Athletic Hall of Fame Nomination Form

    Criteria for Nomination 1. 2. 3. 4. 5. Must have graduated from OHS at least 10 or more years ago. If being nominated as a coach must have had a long distinguished career in Ossining as a coach, with the bulk of coaching experience with the Ossining School District. Be a person of good moral character throughout life. It is a persons accomplishments in athletics, not how many sports the person participated in. The address of the person, or his/her family must be available.

    Sports played at Ossining High School (include details, awards, and accomplishments if known)

    Significant contributions other than athletic involvement while in high school:
