Following points were observed & discussed at site: Sl.N Area o. 1 Boiler#1 Location 12M Observation Welding booth having ten welding machine,electrical panel and other electrical accessories was without fire extinguisher. Welding work was going on at floor without Personal injury/Fire any protection and arc, spatters were directly falling below upto 12m Level. Permanent stair and landing platform was Fall hazard found unprotected Cleaning work of mini turbing was going on in Entrapment/Fall of load suspended position without adequate support. Four chain pulley block was being used as a Unsafe lifting. replacemen of wire rope slings in inclined position for lifting of mini turbine . No proper procedure or JSA was found at site for safe lifting. Core protrusion was found in the wire rope sling for lifting of Mini turbine. Unsafe lifting.

near miss, legal 4 RCC sleepers were being shifted through hydra. During the process 1 sleeper slipped out the chain sling. Endless chain sling was built at site by welding the links. Its test certificate was not found & cannot be matched Fall of material It was observed that buck stay and several material hanging with single lacing.No extra protection and secondary protection provided.were engaged in material shifting Work at height Worker work on the edge of platform without railing/full body harness. Poor cable management was observed as electrical welding cable and electrical cable were entangle on the access.

sub standard working platform was used for Work at height height work.It was not spacious. No safe access and egress was provided to Work at height reach the work location. Huge gap was found in working platform.
