THIS AGREEMENT is entered into as of this day of in Location by and between Discovery Kidzone Montessori School Rachel Supalla (hereinafter referred to as "Provider") and _________________________ (hereinafter referred to as "Parent/Guardian"). This Agreement contains the terms agreed upon between Provider and Parent/Guardian for the care of: Child _____________________________ D.O.B. _____________ Child _____________________________ D.O.B. _____________ 1. Registration/holding Fee: Parent/Guardian agrees to pay non refundable registration/holding fee of $100.00 per child Parent/Guardian agrees to pay activity fee of $10.00 due with first months payment. Security/Holding Deposit Paid [ ] YES [ ] NO Check#___________ Date_________ 2. Monthly Rate: Payment is due and payable the first of every month. If Child is absent from care the normal monthly rate will be due and payable. A two week notice is required before the end of contract is ceased, if not received tuition will be due and payable. Tuition is an annual fee broken into 10 equal payments. Payments Due From August 2012June 1st 2013 The Monthly rate for 2 half days a week will be $140.00 (10% off the second child) The Monthly rate for 3 half days a week will be $190.00 The Monthly rate for 5 half days a week will be $290.00 The Monthly rate for 5 full days a week will be $560.00 The Before/After school care is available from 7:30-8:30 and /or 3:00-4:00 [ ] Option A: up to 1 hours a day $80.00 a month [ ] Option B: up to 2 hours a day $100.00 a month 3. Days and Hours: The parties to this agreement have agreed to the following schedule of care. The school year will follow a traditional school schedule. Tuesday Thursday [ ] 8:40-11:30[ ] 12:00-2:50 Monday Wednesday Friday [ ] 8:40-11:30[ ] 12:00-2:50 Monday Tuesday Wednesday Thursday Friday [ ] 8:40-11:30[ ] 12:00-2:50 Before school [ ] 7:30-8:40 [ ] 8:00-8:40 After School [ ] 2:50-3:30 [ ] 2:50-4:00 4. Late Fees: Parent/Guardian agrees to pay a late fee $5.00 a min for the time that Child remains in care after the hours. If the monthly rate is not paid by the 5th of every month the Parent/Guardian agrees to pay a late fee in the amount of $10.00 per day until the account is current. The check bouncing fee is $35.00 and any other fees it may incur. All late fees are due and payable immediately. 5. Provider Vacation: The parties agree that Provider will be paid for 2 weeks Christmas vacation and one week Easter Vacation and all federal and school holidays that will incur each year. Some dates may be subject to change. The parties hereto have executed this Agreement as of the date and year first above written with Discovery Kidzone Montessori School.
