EVALUATION

HPN

1. Inability to make decisions with respect to taking appropriate health action due to: a. Fear of consequences of action, specifically economic consequences b. Inaccessibility of appropriate resources for care, specifically cost constraints or economic / financial inaccessibility 2. Inability to provide adequate nursing care to a member suffering from rectal cancer due to inadequate resources for care, specifically financial constraints. 3. Failure to utilize community resources for health care due to: a.Fearof consequences of the therapeutic and rehabilitative actions, specifically financial consequences. b. Inaccessibility of required care / service due to cost constraints c. Lack of or inadequate financial resources

After nursing intervention s, the family will avail itself of the necessary medicine for Mr. X hypertension , and Mr. X state will improve within one months time.

After nursing intervention, the family will: a. find other sources of income; b. provide Mr. X with medicine for her hypertension

1. Discuss the consequences of untreated hypertension. 2. Discuss with the family the courses of action open to them, specifically: a. explore the possibility of arranging the schedule of all members such that Mrs.X will always have a caregiver around, the children will have someone there to watch them and either Mr. X can work a double job or Ariceli can start working; b. explore with the family better ways of budgeting family income.
