Roche Accu-Chek Performa ERROR CODES

The meter automatically performs electronic self-tests for battery condition, proper heating range and lighting on each test, assuring the user that the device is working properly. If an error occurs while testing, the meter beeps once (if beep tone has been turned on). Display Check To test the display, turn off the meter, then press and hold the button. The meter turns on and all the display elements are displayed. If any of the display elements are missing, please contact your local device supplier for technical assistance. Error Error: Blank Display Description/Solution The battery is dead; The temperature is too cold; The display is damaged or the meter is defective.

Solution Insert a new battery as described in the Accu-Chek Performa Operators Manual. Move somewhere warmer and repeat the test.

Error: Low battery

The battery is running low; the battery is dead.

Solution Insert a new battery as described in Accu-Chek Performa Operators Manual.

Error: Code Chip / Memory

There is no code chip in the meter.

Solution Insert the code chip into the meter. Make sure the code chip number on the key matches the number that is printed on the test strip container. Make sure time and date are set.

Error: LO Measurement Blood sugar may be lower than the measuring range of the system (10mg/dL; 0.6mmol/L)

Solution Run a quality control test to make sure values are within the expected range. Repeat the test with a new test strip. If the LO result is displayed again, follow your organisations policy. Error: HI Measurement Blood sugar is higher than the measuring range of the system (600mg/dL; 33.3mmol/L)

Solution Run a quality control test to make sure values are within the expected range. Repeat test with new test strip. If the HI result is displayed again, follow your organisations policy. Error: E1 - Test strip The test strip is not inserted properly

Solution Re-insert the test strip, with the arrowheads pointing towards the meter until it clicks into place. Do not bend or move the test strip during the test. If the test strip is damaged, remove the test strip and run test on a new test strip. Make sure the code key number matches the code number printed on the code strip container. Error: E2 - Code Key Incorrect code key in the meter; meter cannot read the code key.

Solution Turn off the meter and remove the code key and insert the new code key that came in the container with the new test strips you are using. Make sure the code key number matches the code on the test strip container. Remove the code key and re-insert the key into the meter. Error: E3 Measurement / Test strip Extremely high blood glucose; Test strip error.

Solution Run a quality control test to make sure values are within the expected range. Repeat test with new test strip. If the HI result is displayed again, follow your organisations policy.

Error: E4 Insufficient blood / control solution Not enough blood / control solution was applied to the test strip; Blood / control solution applied too soon.

Solution If insufficient blood is applied to the strip, you have 5 seconds to re-dose the strip. Discard the used test strip and repeat the test with a new test strip. Error: E5 Code key Code key is from an expired lot of test strips.

Solution Ensure the code key number matches the code printed on the test strip container. Make sure the time and date in the meter are correct. Do not test blood glucose with test strips that have expired. Error: E6 - Test strip Blood / control solution applied too soon.

Solution Repeat the test with a new test strip; making sure it is inserted correctly, with the arrowheads facing towards the meter Repeat the test and once the flashing blood drop appears on the screen, prick your finger and apply a larger hanging drop of blood to the end of the test stick. Do not bend or move the test strip during a test. Error: E7 Electronic error An electronic error occurred; A used test strip was removed and reinserted.

Solution Remove the test strip and turn off the meter. Repeat the test with a new test strip. If the error message appears again, do not use the meter. Error: E8 - Ambient temperature The temperature is above or below the proper range for the meter (10o - 40oC)

Solution Allow the meter to warm up or cool down to room temperature. Do not try to speed up warming or cooling of the meter, e.g. placing it on a heater or in a refrigerator.

Error: E9 Low battery The battery is almost out of power. Solution Immediately change the battery.

Error: E10 Time and Date

The time and date settings may be incorrect

Solution Make sure the time and date are correct and adjust, if necessary, as described in the Accu-Chek Performa Operators Manual.
