Asked what a successful test looks lihe - did anlone say - No. Just do a negative test for 30 minutes.I have neverseenprocedures this. for The volurne of the pill rvas450 barrels. Crews were always on the ball. The visitors did not take any focus away from the well. Steve askedfor help to annotate the charl and Don agreed. They shut in after the pill was above the stack. They were bleeding offto Haliburton. They bled down and it came back up. 1400 psi was the difference between the mud and the seawater. I talked to Hafle about the 1400 - said that if there had been a kick in the well we would have seenit. when I first went to rhe rig floor we talked abour the 1400 psi for a long time - they (TO Toolpusher etc) found it kind of humorous that I talked about it for a long time. Asked about the TP crews - They were experienced,competent hands. ll/ho was on the rigfloor - Randy Ez, JasonAnderson, Bob Kaluza, Driller, Mud Engineer,l-eo and Gordon(they (rnud Eng) were changingout. charles wasn't there. I then n'ent back to the office to look at some paperwork. I had beenthere around l0 - 15 minutes when Jasoncalled - he said we are getting mud back and we are diverting to the gas buster. I grabbed my hat and staned forthe floor it must have taken around 30 secondsro get outside. I went through the short hall and upstairs there was mud and seawater blowing everywhcre, thcrc was a mud film on the deck. I decided not to continue and came back across. Therewas then an explosionI ablast,l hunkereddown on the deck. i was on the port sidethe blast was back .. I then proceeded the rvay to the pipe rack, got a life vest and went to the bridge to all seeifthey had hit the emergency disconnect. I wanted to get the power shut off but ir went off - I don't recall if the power went off before or after the blast. I was glad ir was off. I went to the bridge - the subsea hand said we shoulddisconnect we went to the panel and only the lower annularwas closed. Chris then hit rhe button. It looked like it hit conectly - it all lit up. Saw the gallon count. Chris said he saw the stabretract. I was then talking to peopleand gening them into the capsure therewere some injured people in the capsulesI stayedlong enoughto get everyonein the capsure, someguysjumped with a raft. The last guy came in and we olosedthe door.

I didn't seeany flames when the explosion occurred. The weather was calm. We hadn't back loaded all the rnud - I told the mud engineerto keep some. I heard people say that the engine room was gutted. We then went to the Bankston. |las asked if it had been mud or spacer that was being sprayedfromthe might have been spacer,my feeling is there was some mud. well - it

Never indicated any inoease in flow! I spoke to the mud logger - he didn't see anything unusual. The mud engineerwas watching the returns. Don was asked - what do you think happened - | have no idea! AdditionalThe crervs were good. The diverter can go overboard or to the poor boy degasser. When Jasoncalled to say he was diverting flow to the poor boy degasserhe also said he either was closing or had closed the bag (annular) Don thought the diverter was not closed - he said it appearedthat fluid was coming from the rig floor. He estimatedit took him I or 2 minutesto get outsidewhen he got the call. Don did not know of any issues with the BOP system. Asked about being spacedo,tt for ,he BOP - Don said that they were good about spacing out but didn't check. Asked about block height - didn't know - said he would expect them to be at the spaceout position. Didn't know if the shears would cut the tooljoint. Didn't know if there was I or 2 overboardlines on the diverter. Stated - I don't know why that mud got so far up the riser. ClariJied - When the toolpushercalled he didn't say it was a blow out or kick, mud was coming back and he was diverting to the gas buster_ on the boat the senior Toolpushersaid the AD had called to say they had a well control problem. when I rvent outsideI w,as wonderingwhy they had not soundedgeneralquarters I realisedlater that the fire alarmswere soundins.

Don Vidrine Interview - Tuesday27th Anril Don was working the night tour and hadjoined the DW Horizon team during January this year (2010).He saidthis was his 4th trip. The hole had alreadybeendrilled when he arrivedto starthis trip. There had beenkicks and losses, mud weight had beencut andthey were closeto the balance. The loggingprogramwent ok, sometool problems, hole problems. no on the wiper trip after loggingpumpedout, because they werecloseto balance, the to top of the 11718"(latersaidthe top of the 9 7/8") Gas readingwas 1000units at bottomsup, this is not uncommon,it stayeda little longerbut we hadbeenout of the hole for a week. Pumpedout" took the correctfill up, to top of 9 7/8"'liner, good trip. Ran the 9 718x 7" casing, trip went well. Cleared pipe and circulated the retums(lOGr bbls)to the top of the liner. cementjob went ok, complexjob with differentfluids, lookedrike it went fine, bumpedthe plug andtested sealassembly. the Pulledout of hole to pick up the cementstinger. Ran in hole to the top of the BoPs and testedthe casing,very good test basedon the charthe saw. Carriedon runningin the hole to depth,riggedup and started displacement. a the Had procedure, displaced chokeand kill lines,pumped disptacement followed the the pill by water. conversation thenwent to Don's arrival at the rig floor to starthis tour. when he arrivedtherewas 1400psi on the Dp. He saidthey shutthe Dp back in and it had comebackup. He questioned andwastold by the teamin the drillers this doghouse this wasannularcompression he had heardaboutthis but had not seen that He went to the kill line and therewasno pressure. They thenpressured on the kill line to checkit was full - it was lined up to the up hydraulicchoke. Opened kill line and it was staricaI30 psi. the

Openedthe choke to the mini trip tank and bled off 2ll0s (two tenths)of a banel. It slowedto a drip then stopped. Watchedfor 30 minuteswith no flow. The 1400psi remainedon the drill pipe, it stayedat that pressure there was no indication that it was gascoming up. Initially therewas 2400psi on the drill pipe - this wasthe differentialon the u tube and 60 barrelswere bled off. The 1400psi was the differencebetweenthe mud in the riser- This was annular compression they (TP etc) said it doesthat all the time. If we have 1400psi on the drill pipe we shouldseeit on the kill line? Let's bleedit off and see- the kill line was bled then stopped. I then went to call Hafle when I cameback they were still watching the strippingtank and the dripping had stopped. Everything looked fine. we then closedit in andopened bag (annular).Got the pumpslined up. Told the them to let me know when the pill comesup so that we could do a sheentest. I thenwent to the office and checked what callsI had. They calledto saythe pill was backso I went to the rig floor. Everythingwas fine, checkedfor flow and it was ok. Had takensomespacer was l6ppg. Everythinglookedfine. with the spacer it in there,therewas still an 8 - 900 psi underbalance. The only otherthing is that if they hadalreadyopened dump line??- I didn't go the down there. The strokeson the panel looked aboutright - everythingwas fine when we were doingthe sheen test. The sheen waspassing. test said to go aheadand start dumping,told them everybodyget in place and start dumping. The plan was ro circulate thenwatch it for 30 minutes Reverted backto negative test. The Tourpusher and SeniorToolpusher told me it wasthis annularcompression thing. I wantedto do another test- I don't know why we didn't seepressure the kill on line?? Asked about procedurefor negativetesting - There is no standard procedureit is left for the rig to figure it out.

Asked what a successfultest looks like - did anyonesay - No. Justdo a negativetest for 30 minutes. haveneverseenprocedures this. I for The volumeof the pill was 450 barrels. Crews were always on the ball. The visitors did not take any focus away from the well. Steve askedfor help to annolate the chart and Don agreed. They shut in after the pill was abovethe stack. They were bleedingoff to Haliburton. They bled down and it cameback up. 1400psi was the differencebetweenthe mud and the seawater. I talked to Hafle aboutthe 1400- saidthat if therehad beena kick in the well we would haveseenit. When I first went to the rig floor we talked aboutthe 1400psi for a long time - they (TO Toolpusheretc) found it kind of humorousthat I talked about it for a long time. Asked aboul the TP crews - They were experienced, competenthands. lYho was on the rig floor - Randy Ez, JasonAnderson,Bob Kaluza, Driller, Mud Engineer, Leo and Gordon(they (mud Eng) werechanging wasn'tthere. out. Charles I then went back to the office to look at somepaperwork. I had beentherearound l0 - 15 minuteswhen Jason called- he saidwe aregettingmud backandwe are diverting to the gasbuster. I grabbedmy hat and startedfor the floor it must havetakenaround30 seconds get to outside. I went throughthe short hall and upstairstherewas mud and seawater blowing everywhere, therewas a mud film on the deck. I decided to continue not and camebackacross.Therewasthen an explosion a blast,I hunkered / down on the deck. I wason theportside blast the wasback. ............ I then proceeded the way to the piperack,got a life vestandwent to the bridgeto all seeif they had hit the emergency disconnect. I wantedto get the powershutoff but it went off - I don't recallif the powerwent off before or after the blast. I was glad it was off. I went to the bridge- the subsea handsaidwe shoulddisconnect we went to the panelandonly the lower annularwas closed. Christhenhit the button. It lookedlike it hit correctly- it all lit up. Sawthe galloncount. Chris saidhe sawthe stabretract. I was thentalking to peopleand gettingthem into the capsule therewere some injuredpeoplein the capsules. I stayedlong enoughto get everyone the capsule, in someguysjumped with a raft. The lastguy camein and we closedthe door.

I didn't seeany flameswhenthe explosion occured. The weatherwas calm. We hadn't back loadedall the mud - I told the mud engineerto keep some. I heardpeoplesay that the engineroom was gutted. We then went to the Bankston. rl/as asked if it had been mud or spacer that wss being sprayedfrom the well - it might have beenspacer, feeling is there was somemud. my Never indicatedany increase flow! I spoketo the mud logger- he didn't see in anything unusual. The mud engineerwas watching the returns. Don was asked- what doyou think happened- lhave no idea! AdditionalThe crewswere good. The divertercango overboard to the poor boy degasser. or When Jasoncalled to say he was diverting flow to the poor boy degasser also said he he eitherwasclosingor had closedthe bag (annular) Don thoughtthe diverterwas not closed- he said it appeared fluid was coming that from the rig floor. He estimated took him I or 2 rninutes get outsidewhen he got the call. it to Don did not know of any issues with the BOp system. Asked about being spacedoutfor the BoP - Don said that they were good about spacingout but didn't check.
