PYSCHOLOGICAL REPORT I. Identifying information.

II. Reason/s for referral. (5 points) Write the reason/s why the client is to be assessed. What purpose does this assessment will serve? Or the importance of assessing the client in connection with his/her present conditions or needs. Cite the person who, if there is, referred the client for assessment (i.e. teacher, supervisor, clinical psychologist, or a any particular person). Feel free to write the name of the person who refers the client unless he/she (the person who gave the referral) asked you otherwise. III. Test administered and date of administration. (5 points) Present the battery of tests administered and its date. This could be presented in tabular form. IV. Observation. (10 points) In this part, behavior of the client will be discussed in the whole duration of test administrations. Discuss also the affect (emotions and feeling) of the client. Do not assume or interpret the affect of the client (do not give reason/s why he/she feels that way). The attitude of the client towards the test and maybe to the examiner should also be discussed. Discussion in the observation should not be done per test, meaning it should be a general observation. Certain events could be cited but need not requires to mention the type of test being administered. V. BACKGROUND (10 points) Provide a background of the client that may be relevant in the conduct of the assessment. Particularly, include information concerning previous behaviors and achievements in school (especially for mental ability assessment) and social relationships with people in various areas (family, peers, etc) (especially for personality testing) VI. Test results and interpretations. (15 points) Based on the test scores, you now look at the capability of an individual. Point out the weaknesses and strengths of the person in relation with the reason/s why the client is being assessed. Do not anymore discuss in the interpretation the numerical value of the tests, instead focus on the implication of these scores on the behavior of the individual. At the same time, do not discuss the test results individually; look at the interconnections of all the tests and come up with a holistic psychological description of the client. If you did not conduct an interview, do not add information (you may know the client personally) aside from the test results. VII. Summary, Conclusion, and recommendation. (10 points) Summarize the assessment procedure. Briefly discuss parts 1-4. From the interpretations, make an impression regarding the clients conditions. With all these pieces of information that you have regarding your client, make a necessary recommendation/s. As an evaluator, what would you professionally suggest of the things that the client should do? Do not anymore recommend to re-test a client who has already taken a battery of test. You could recommend a person to see a counselor, clinical psychologist, attend self-enhancement seminar, undergo a therapy, resolve personal issue through professional help, and engage in activities that would enhance psychological well-being. These are only examples that an examiner could suggest; however elaboration of the purpose should be made. In addition, more concrete recommendation would be advised than abstract ones. Recommendations also vary from case to the other.
