PLEASE NOTE: If registering for the on-line video streaming Preparation Program option (Distance Program with instructor support), please indicate prep centre code 71. Please note that all students whether enrolled in the lecture or distance Preparation Program option will be required to write the Mock Entrance Examinations at one of the supervised Examination centres. Candidates registering for the Preparation Program via distance will be placed in an exam centre in the same region where they have selected to write the Entrance Examination. Students who are unable to write the Mock Entrance Examination at one of the supervised centers on the scheduled examination date(s), and would like a copy of the Mock Examination(s) mailed to them, will be responsible for the courier charges associated with the mailing.

Payment must accompany this application. All fees quoted are inclusive of HST (Registration No. 12396 9610 RT0001). Material will be shipped when available after receipt of application form and full payment. Ensure that you are familiar with the cancellation and refund policies below prior to submission. Location confirmations will be available on-line individually for the Exam Preparation Program (if registering for a Lecture centre) and the Entrance Examination approximately 2 weeks prior to the event. Please note:
Cancellations for the Entrance Examination and Preparation Program are accepted only when the requests are received no later than April 15, 2011. Requests must be submitted in writing and will be subject to an administrative fee of $350, even if the cancellation is submitted in advance of the enrolment deadline date. Material may not be returned for credit. Spaces in the Entrance Exam Preparation Program lecture centres are allocated on a first-come, first-served basis upon receipt of completed applications. All Preparation Program lecture centres have maximum enrolment capacities, so register early to avoid disappointment. Dates and locations are subject to change, and are subject to minimum enrolment.

1. Professional Misconduct (1) In this section, negligence means an act or an omission in the carrying out of the work by a Member that constitutes a failure to maintain the standards that a reasonable and prudent Member of equivalent designation in CMA Ontario would maintain in the circumstances. (2) For the purposes of the by-laws, professional misconduct means: (a) negligence; (b) a breach by a Member of the Act or the by-laws; (c) undertaking work the Member is not competent to perform by virtue of his or her training and experience; (d) the Member has: (i) (ii) committed or been convicted of any criminal offence including but not limited to fraud, theft, forgery, tax evasion, and violation of securities laws; or pleaded guilty to or been found guilty of a criminal offence but has been discharged absolutely or upon conditions prescribed in a probation order;

(e) the failure of a Member to respond promptly and cooperate fully with respect to requests for information and other communications from the Society. 2. Code of Professional Ethics All Members will adhere to the following Code of Professional Ethics of CMA Ontario: (1) A Member will act at all times with: (a) responsibility for and fidelity to public needs; (b) fairness and loyalty to such Members associates, clients and employers; and (c) competence through devotion to high ideals of personal honour and professional integrity. (2) A Member will: (a) maintain at all times independence of thought and action; (b) not express an opinion on financial reports or statements without first assessing her or his relationship with her or his client to determine whether such Member might expect her or his opinion to be considered independent, objective and unbiased by one who has knowledge of all the facts; and (c) when preparing financial reports or statements or expressing an opinion on financial reports or statements, disclose all material facts known to such Member in order not to make such financial reports or statements misleading, acquire sufficient information to warrant an expression of opinion and report all material misstatements or departures from generally accepted accounting principles.

(3) A Member will: (a) not disclose or use any confidential information concerning the affairs of such Members employer or client unless acting in the course of his or her duties or except when such information is required to be disclosed in the course of any defence of himself or herself or any associate or employee in any lawsuit or other legal proceeding or against alleged professional misconduct by order of lawful authority of the Board of Directors or any committee of CMA Ontario in the proper exercise of their duties but only to the extent necessary for such purpose; (b) inform his or her employer or client of any business connections or interests of which such Members employer or client would reasonably expect to be informed; (c) not, in the course of exercising his or her duties on behalf of such Members employer or client, hold, receive, bargain for or acquire any fee, remuneration or benefit without such employers or clients knowledge and consent; and (d) take all reasonable steps, in arranging any engagement as a consultant, to establish a clear understanding of the scope and objectives of the work before it is commenced and will furnish the client with an estimate of cost, preferably before the engagement is commenced, but in any event as soon as possible thereafter. (4) A Member will: (a) conduct himself or herself toward other Members with courtesy and good faith; (b) not commit an act discreditable to the profession; (c) not engage in or counsel any business or occupation which, in the opinion of CMA Ontario, is incompatible with the professional ethics of a management accountant; (d) not accept any engagement to review the work of another Member for the same employer except with the knowledge of that Member, or except where the connection of that Member with the work has been terminated, unless the Member reviews the work of others as a normal part of his or her responsibilities; (e) not attempt to gain an advantage over other Members by paying or accepting a commission in securing management accounting or public accounting work; (f) uphold the principle of adequate compensation for management accounting and public accounting work; and

(g) not act maliciously or in any other way which may adversely reflect on the public or professional reputation or business of another Member.

(5) A Member will: (a) at all times maintain the standards of competence expressed by the Board of Directors from time to time; (b) disseminate the knowledge upon which the profession of management accounting is based to others within the profession and generally promote the advancement of the profession; (c) undertake only such work as he or she is competent to perform by virtue of his or her training and experience and will, where it would be in the best interests of an employer or client, engage, or advise the employer or client to engage, other specialists; (d) expose before the proper tribunals of CMA Ontario any incompetent, unethical, illegal or unfair conduct or practice of a Member which involves the reputation, dignity or honour of CMA Ontario; and (e) endeavour to ensure that a professional partnership or company, with which such Member is associated as a partner, principal, director, officer, associate or employee, abides by the Code of Professional Ethics and the rules of professional conduct established by CMA Ontario.
