2011-12 Evaluation Questions & Answers

Question: I have an employee on my list that I am supposed to evaluate. However, that employee is currently on a long term leave or was on leave for a significant portion of the school year which prevented me from observing the employee/completing an annual evaluation. How do I complete an evaluation for him/her? Complete the name/date portion of the appropriate evaluation form for that employee. You do not have to go through and fill out every section (if you are not able). However, you should fill out the comments section and state _(name)_ was on leave from _(date)_ to (date the date you are completing the evaluation). Due to the employee being on leave I was unable to provide the employee an end of the year evaluation. Question: evaluation? Complete the appropriate evaluation for that employee and add his/her name and information to the roster. Question: Who evaluates an employee who is part time in my building and part time in another building?

According to the language of the certificated collective bargaining agreement, a certificated employee who serves equal time in two (2) buildings may receive two evaluations or only one evaluation depending upon the preference of the employee. Please check with the employee and his/her supervisor in the other building to ensure that the employee receives an evaluation. A certificated employee who is assigned to more than two buildings by central administration will be evaluated by his/her central administration supervisor, with input from the building principal or his/her designated administrator. Question: What do I do if an employee on my list is not in my building?

Contact your Employee/Labor Relations Manager or Elaine Williams so they can problem solve who should be completing that employees evaluation. Question: What do I do if an employee on my list has resigned or retired before the evaluation due date?

Complete the name/date information on the appropriate evaluation form for the employee and state that they resigned/retired in the comments section. Also specify the date that the employee separated from the District. Question: Why do I need to complete the provisional evaluations so soon April 13?

The April due date was created to make sure that Employee/Labor Relations is aware of any certificated employees on provisional contracts who, because they have not met the performance schedule, are not eligible for a renewed contract. See Article XI, Section D.5.b of the CBA. If you have performance concerns about a provisional employee, please let your Employee/Labor Relations Manager know as soon as possible. Question: What happens if the employee refuses to sign the evaluation form?

Remind the employee that his/her signature acknowledges receipt of the evaluation and does not denote agreement with the content of the evaluation. If the employee still refuses to sign, make a note on the form that the employee was provided the evaluation on (insert date) and refused to sign. Send the form in with your other completed evaluations.
