Personal Information Name?


Legal name is the name that appears on your documents (drivers license, passport, etc.) Commonly used name is the name that you use in practice and in public. If you use a Chinese name and an English name, you must include both in your application. If more than one names, you must provide documents to indicate you are the same person OR have this page notarized by a lawyer.

Select the name you want to appear on your certificate of Registration. The name(s) you choose are the name(s) you will use in your practice. In this example, the name that would appear on the Certificate of Registration would be John Lee Chow because BOTH was selected. If John only wanted to use his legal name, his name would appear as Lee Chow on his Certificate of Registration. If only Commonly Used Name was selected, the name that would appear would be John Chow.

Language Fluency
If you are not fluent in English or French, you must complete a language plan that outlines how you will communicate with patients, other health care providers and emergency services. If you completed the Jurisprudence test with translation assistance, you must complete a language plan.

Language Plan Example


When treating a patient who speaks English, I will have my co-worker who is fluent in English act as translator. In the case of emergency, my co-worker will speak with 911/emergency services.
In addition, I am taking English lessons at the Chinese Cultural Centre to improve my English fluency.

This is an estimate. Indicate the estimated time frame in which you completed 2000 patient visits. This number will vary from practitioner to practitioner depending on how busy your clinic is, etc.

Patient Visits - Estimate


Go through the list of disorders and indicate if you have diagnosed that disorder. Indicate the estimated number of patient visits : herbal only / acupuncture only / herbal and acupuncture Again, the numbers are estimates. The number of total visits of herbal only, acupuncture only and herbal and acupuncture should equal to the grand total of visits.

Validation
The witness cannot be a member of your family. It can be anyone who knows you in your profession as an acupuncturist / traditional Chinese medicine practitioner. Fellow practitioners or co-workers can act as a witness.

Liability Insurance
Make sure you put an x/checkmark in the first box indicating that you will comply with the Colleges requirement.
If you do not have professional liability insurance, make sure it will be in place by April 1, 2013. Not having a current professional liability insurance is against the law.

Declaration of Conduct
Be completely honest when filling in the declaration of conduct. Failure to disclose information covered in the declaration will result in delays in processing your application. Example: John was found guilty of fraud when he was a young man. He has to disclose this to the College, so he would select YES for a). John includes a letter with his application briefly explaining his past conviction.

Impairment
If you are currently suffering from, or have previously suffered from, a physical or mental condition or disorder that you are being treated for, you must disclose it to the College. The College will contact your Doctor/the regulated health professional treating your condition. This could include mental health conditions and physical conditions.

Example, John has been diagnosed with bi-polar disorder. During manic states, John becomes extremely animated and is not safe to practice acupuncture. However, John has been receiving treatment for his disorder and he has not had a manic episode for over 5 years. He selects YES to question i) and discloses this to the College. His doctor includes a letter explaining that John has been successfully treated for his disorder and is safe to practice as long as he continues to take medication.

For most applicants, the answer to these questions will be NO.


If you answer YES to any of the questions, include a letter explaining the circumstances and details of the case. Failure to answer these questions honestly will result in a delay in the processing of your application.

Criminal Background Check


Depending on your location, a criminal background check can take 1 day to 3 weeks.
Commissionaires can also complete a criminal background check in approximately 48 hours. Check www.commissionaires. ca for locations.

If you were apprenticed without a diploma, we suggest you attach a letter of recommendation from your teacher(s) indicating how long you have studied.

Double check that all sections are filled in completely. If there are sections that do not apply to you, write N/A in the blank space. Photocopy or scan your completed application and keep a copy in your personal files.

Make sure that you have included: Jurisprudence and Safety Program letter; proof of identity; proof of professional liability insurance; passport sized photo; language plan (if needed) and criminal background check
