BP Press conference 09/10/2010

No single factor caused the Macondo well tragedy. Rather, a sequence of failures involving a number of different parties led to the explosion and fire which killed 11 people and caused widespread pollution in the Gulf of Mexico earlier this year. A report released by BP today concludes that decisions made by multiple companies and work teams contributed to the accident which it says arose from a complex and interlinked series of mechanical failures, human judgments, engineering design, operational implementation and team interfaces.

ELIMINATION The hazard is removed altogether. SUBSTITUTION Replace the hazardous system or process with one that presents a lower risk. ENGINEERING CONTROLS Structural change to the working environment or work process that forms an additional protective barrier between the hazard and the employee. ADMINISTRATIVE CONTROLS Reduce exposure to hazards through procedure, Instructions, training and competency. PERSONAL PROTECTIVE EQUIPMENT (PPE) Worn by exposed employees to provide a last line of Defence should other controls prove ineffective or used Together with other control measures. The protection offered by PPE relies on the correct selection, fitting, Maintenance and use.

Biggest challenge

2.5 Injury Analysis 2.5.1 The biggest Cause of accidents/incidents is Struck by 29.82% followed by Caught in between 24.56%. 2.5.2 The Job Category involved with the above is Trammers/Chute Operators 85.71% and Drillers 14.26%. 2.5.3 The Body Part most effected is hand 57.1%.
