Accident at Michigan
Plastics Plant Claims
Life of Worker
Sat 5:11 PM, Jun 28, 2014
An employee of Grand Rapids Plastics Inc. died following an industrial
accident involving an injection molding machine. An employee was
polishing the machine June 27 when another employee, not knowing he
was inside, turned on the machine, local news reports. The employee
died from his injuries. The Michigan Occupational Health and Safety
Administration is investigating the incident.
Root Cause:
1.Employee was working in a confined
space
2.Employee was not wearing work
gloves while making the adjustments.
3.There are no SW documents requiring
anyone to wear gloves while making
adjustments to machinery or
equipment that is not running.

What Occurred: An employee was making an adjustment to the side
pusher in the auto pack module on line 315. When he loosened the
allen bolt to make the adjustment, the allen key slipped and his first
knuckle on his middle finger hit the braker and lacerated his knuckle.
Edge where finger laceration occurred.
Root Cause: 1) Housekeeping was
an issue The area was cluttered and
a box was left on the floor. 2)
Employee tried to take a short cut
and walk through a narrow area not
designated as a walkway. 3)
Employee was focused on getting the
attention of the lead and not where
the employee was walking.
Location: Presto-Tamaqua Date: 09-JUN-14 Injury: Fracture
What Occurred: An employee was working on line 130 and was picking
up scrap from the floor. Employee noticed product rolls coming off the
machine and was trying to get the attention of her lead to ask if she
should pack the product. She started walking towards her lead and
tripped over a box of sleeves on the floor. She tried to catch the fall and
her arm was caught on the pallets.

Root Cause: 1) Employee was
lifting film from floor to the gaylord
(46). 2) Employee stated he
incorporated proper lifting
techniques using his legs to lift
scrap. 3) Employee may have lifted
to much film at one time (60 lbs).
Location: Presto-Tamaqua Date: 24-JUN-14 Injury: Strain
What Occurred: An employee was changing a roll on Line 300, had to
cut down the roll due to defects in the film. Employee bent down to lift
the film from the floor and as the employee lifted up, he felt a twinge in
his lower back. Employee paused and then tried to walk and was
unable to put any weight on his right leg. He then called for assistance
from the lead. Employee estimates he was lifting approximately 60 lbs

Location: RPG- Hot Springs Date: 26-JUN-14 Injury: Strain
What Occurred: A caster operator was crimping the seal on
steel banding on a coil of aluminum using the manual
crimping tool when he felt a pop and sharp pain in his right
shoulder. The employee could barely raise his right arm
afterwards. The employee stated he was using the crimping
tool at chest height.
Root Cause: The force
used to crimp the seal
around the steel banding
caused the caster
operator to suffer a
shoulder strain.
Location: RPG- Louisville Date: 30-JUN-14 Injury: Amputation
What Occurred: A caster employee was reattaching a crane hook to its
grab. As he got the hook and grab aligned, the employee slipped on
the platform. As this happened, the employee reached out and
grabbed with his right hand, the stop on the stand. (large red arrow)
As the employee grabbed the stop, the employee running crane,
connected to the grab began to lift the connected units up. In doing so
employee that grabbed stop, got his 3
rd
finger pinched between the
spreader bar and the stand (small red arrow). .
Root Cause: (1) Employee was using the small side of the Christmas
tree rack that is not designed to step up on and his foot slipped (Yellow
Arrow). This extension is only 6 inches wide. The work should have
been done on the opposite side of the Christmas tree as per design. (2)
The platform that was supposed to be used, was being used as a
storage area for safety barricades for roll change. (Green arrow). (3)
Housekeeping on the Christmas tree was not properly managed.
