REMEDY
1.

Those identified as MAJOR ERRORS


shall NOT BE ACCEPTED unless the
entries are CORRECTED, thereby
maintaining the NO DEFFERED
Policy.

2.For Major Errors a court petition must


be filed before the Board Exam.

REMEDIES for MINOR ERRORS


Those identified as MINOR ERRORS
in the usual writing or printing of names,
with or without hyphen, whole or separate
names, whole or abbreviated traditional
names, discrepancies in vowel/double and
single letters in the names SHALL BE
ACCEPTED

provided an AFFIDAVIT OF
DISCREPANCY shall be executed and
submitted. The name in which the
applicant is customarily known shall
be followed.

Those with wrong gender or without gender


printed on the birth certificate SHALL BE
ACCEPTED provided the following shall
be presented and submitted:
a.Birth certificate LCR Form 1A

authenticated by NSO or voters


registration record

a.Parents/ guardian or personal

affidavit
b.Barangay certification attesting the

applicants true and correct gender

Guidelines in the

1.No entry on the NSO Certificate

of Live Birth
Example:

- No Date of birth

REMEDY
- To submit a photo copy an
annotated Birth Certificate from
NSO, with given name and date of
birth indicated.

To submit any of the following:
- Authenticated copy of LCR Certificate

of Live Birth

- Baptismal Certificate
- Philippine Passport
- Voters 'Registration Record or

COMELEC Certification

MOTHERS MAIDEN NAME INDICATES
MARRIED NAME
REMEDY
To submit ANY of the following:
- Mothers Birth Certificate

- Marriage Certificate of parents


- Mothers Voters Registration

- Mothers Philippine Passport

ADOPTIVE PARENTS
REMEDY
To Submit Birth Certificate of SECPA or
NSO
- Must bear the annotation of the courts

decision and the finality of the


adoption.

Without annotation- submit


a copy of the courts
decision or a certification
on the finality of the
adoption.

BIRTH IN A FOREIGN COUNTRY


REMEDY
To submit ANY of the following:
- Report of birth on SECPA issued by the

Philippine Embassy/ Consulate


- Photocopy of the birth certificate issued by
competent foreign authorities and Alien
Certificate of Registration (ACR)

IMPORTANT

If the report of birth is not on NSO


security paper, the application
shall be advised to go to DFA for
endorsement of the Record of
Birth to NSO for future
requirements

FEMALE MARRIED APPLICANT


WHOSE MARRIAGE SOLEMNIZED IN
A FOREIGN COUNTRY

REMEDY
To Submit
- Report of Marriage issued by the

Philippine Embassy /Consulate in NSO


security paper.

IMPORTANT
if report of Marriage is not yet in the form of
an NSO security paper, applicants are
advised to go to DFA and have it be
endorsed to NSO for future requirements.

NON-READABLE ENTRIES
REDEMY
Must be supported by a clear copy of:
- LCR Form 102 or LCR Form 1A duly
signed/ certified by the LCR
