Recommendations

Need For Project

The agency was in need of a method to track the
status of clinical recommendations from both
Alliance Behavioral Healthcare and the Intake
Clinician to ensure that consumers are receiving
the appropriate care. This form allows a broad
overview of the recommendations for each
consumer including pro-social activities,
psychiatric and psychological evaluations, school
recommendations (IEP/BIP), substance abuse
treatment (Contingency Management), and
Medical recommendations.

The Project
A chart was created that consisted of columns
with each recommendation and rows with each
consumers name. I reviewed each consumers
comprehensive clinical assessment, as well as the
consumers recommendations from Alliance, and
recorded these recommendations on the chart for
tracking purposes. I met with each individual
therapist to notate the status of these
recommendations for each consumer on their
caseload. This chart will provide a visual overview
of each consumers recommendations, the status
of each recommendation, and a comprehensive
way to identify concerns that need to be
addressed.
