Analyze the Evidence


1.Independently look at your data (Homeroom, grades, student work,
assessment data). In a Word document, answer the following
questions:
a. What does the data say the students in your classroom are
struggling with?
b. Which students are struggling with which skills?
2.Independently reflect on your teaching practice. As their teacher for
the year...
a. What instructional practices, strategies, knowledge should you
focus on this year to improve student achievement?
b. What evidence do you have that this instructional

Collaborate
Get into your PLC groups to discuss and
record...
a. What does the data say your kids in your
department struggle with?
b. What learning around instructional practice and
knowledge do you want/need in order to
improve student achievement?

Determine a Focus
1. What is your 5D focus this year?
2. What state criterion will you focus on this year?
3. Work in your PLC to determine your team Student Growth Goal based on the data
a. Which content skill will you focus on improving?
b. Which students will be your focus?
c. What is the goal for the students?
d. What data will you collect to show student progress toward the goal? (remember you need multiple
sources of data - both formative and summative)
e. What are your two points in time?

4. Work independently, with support from your PLC as needed, to determine your 3.1 goal based
on your team goal and your classroom data. Use the same guiding questions that you used to
create the team goal.

Implement and Support


1. What do you need to know in order to
address the student learning problems
and your instructional needs?
2. What resources will you use/need in
order to implement your plan?

Develop your Action Plan


1. What specific actions/steps must be put in place in
order to meet your student growth goal?
2. What evidence will you have that each step is
being completed?
3. What resources do you need (time, space,
equipment, professional development) in order to
achieve your goal?
4. What is the timeline for each action step?

Are there students you could bring in to work with more one on one or
small group?
Is there new learning you need that you can do as a PLC or as an
