3 Extractions 5+ years ago
Brushes at least once per day
Upper left molars sensitive to sweets
Vital Signs:
Blood Pressure: 111/77
Respirations: 12 per minute
Pulse: 76 beats per minute
PRE-TREATMENT DENTAL
ASSESSMENT

Extraoral/Intraoral Findings: Bilateral popping


of TMJ, no pain associated with popping, no
locking of TM joints reported.
Radiographic Assessment/Clinical Assessment
by dentist: number 2 MO restoration, number
14 MO restoration both due to decay;
recommended extraction of teeth numbers 15
and 16 due to decay, recommended extraction
of retained root tip of tooth number 4.
Recommended patient seek further
consultation/treatment for impacted tooth
number 11.
