    Mr. K is a 62-year-old male farmer who presented with a bulge in his left inguinal region that causes pain. The bulge first appeared one month ago and has become persistent. On physical examination, the patient had tenderness over the left inguinal region with a bulging oval mass measuring 6x4 cm. Based on the history and examination, the diagnosis was an irreponible left lateral inguinal hernia. The treatment plan included routine blood tests, abdominal ultrasound, IV fluids, analgesics, antibiotics, and consultation with a surgeon.

    IDENTITY

    HISTORY TAKING

    Chief complain bulge in the left inguinal


    Anamnesis : Suffered since 1 days ago. The bulge feel pain.
    bulges first appeared one month ago like a ping pong ball. The
    bulging rose when the patient in activities and lose when the patient
    take a rest. But now the bulge was persistant, the bulge can not be
    put back. Nausea (+), vomit (-), Anorexia (-), fever (-),abdominal
    pain (+). Normal defecate and urinate. No history of trauma. History
    of prostatectomy procedure one years ago.
    PHYSICAL EXAMINATION

    The patient was conscious with moderate ill.


    BP = 140/80 mmHg
    Pulse = 76x/m, regular, strong
    RR = 18x/m, regular
    Temperature = 36,6oC
    GENERALIZED STATUS

    Abdomen
    Head : Normal
    Inspection : Flat, follow the motion of breath,
    Face : Normal
    scar (+) at the hypogastric region
    Eyes : Normal
    Auscultation : peristaltic was decrease
    Nose : Normal
    Percussion : tympany (+)
    Mouth : Normal
    Palpation : tenderness (+)
    Ears : Normal
    Neck : Normal
    Genitalia
    Chest : Normal
    Inspection : bulging at left inguinal region ,
    Extremity
    oval like a ball with size 6x4 cm . Hyperemis (-)
    Superior : Normal
    Palpation : Warm (-), tenderness (+)
    Inferior : Normal
    Auscultation : persitaltic was decrease
    CLINICAL FINDING
    DIAGNOSIS

    Irreponible left lateralis inguinal hernia
