History For Development Of ISM Code.

On the evenning of March 6, 1987, the cross-channel R0-Ro ferry Herald


Of Enterprise carrying 450 passengers, 80 crew, more than 80 vehicles,
and close to 50 freight wehicles, left the Belgian port of Zebrugge for the
port of Dover.
Soon after the Herald of Enterprise passed the Zebrugge breakwater,
water flooded the ferry lower car deck and destabilize the ship, causing it
to sink in a matter of minutes, 193 lives were lost.
The inquiry report summed up the shore managements cavalier attitude
towards safety.
The general frustration in the shipping industry following the capsizing of
the Herald of Enterprise is a typical of the kind of accident that
precipitated in a paradigm shift in maritime safety administration and the
development of ISM Code.
SOLAS adopted the ISM Code in 1994, and incorporated into the
chapterIX. By 1998 most of the commercial shipping community was
required to be in compliance with the ISM Code. By 2002 almost all the
international shipping community was required to comply with ISM Code.
The ISM Code provide an International Standard for the safe management
and operation of ships and for pollution prevention.
The Master is the authority onboard and he
should be given the overriding authority and
discretion to take such action in the best
interests of the ship on matters relating to
SAFETY and POLLUTION PREVENTION
The Company must employ the right people
onboard and in the office and make sure that all of
them;

Know what their duties are


Receive instructions on how to carry out their
duties
Get trained when and if necessary
The Master is properly qualified for command
The Master is fully conversant with the SMS
The Master is given the necessary management
support
Ship personnel can communicate in a language
understood by all
Ship personnel can read and understand the
relevant SMS information
Objectives of the company Safety
Management System should be;

safe practices in ship operation;


safeguards against all identified risks;
continuous improvement of safety
management skills of all personnel in the
company.
It means every Company should develop,
implement and maintain a Safety
Management System (SMS) which include
the following functional requirements.

A Safety Management System means a


structured and documented system enabling
Company personnel to implement effectively
the Companys Safety and Environmental
Protection Policy and to provide for safe
practices in ship operations.

It may be in the form of a manual, a set of


procedures, fleet instructions, circular letters
or asset of reference documents.
The Company must put in writing its POLICY on
the Safety and the Protection of the Marine
Environment and make sure that everyone knows
about it and follows it.

The Company must have sufficient & suitable


people (in the office and on vessels) with clearly
defined roles & responsibilities (who are
responsible for what).
The Company must appoint a person in the office
responsible for monitoring and following all
SAFETY matters of the vessels.

The Master is responsible to make the System


work onboard.

He should motivate his crew in following the


system and give them instructions where
necessary.

You need to plan your work on the ship and follow your plan
when working.

NOBODY PLANS TO FAIL BUT MANY FAILED TO PLAN.

You should be prepared for the unexpected (emergency).


It can happen any time and any where

The Company should develop plans for responding to


emergency situations onboard its vessels and practice
them
No person or system is perfect

The good thing about this system is that it gives you a way
to correct it and improve
When you find something wrong including accidents and
hazardous situations and near misses, you should report it
When the report is received, it will be analysed and where
necessary the whole system can be improved.
