Physical examination

Antepartum hemorrhage
General features
Hemodynamically stability need to exclude sign of shock e.g. pallor,
sweating, hypotension, tachycardia
Be cautious of concealed hemorrhage shock is out of keeping from
observed hemorrhage
Abdomen
Check for abdominal scars (previous LSCS scar)
Palpation
hard and tender uterus (placental abruption)
Soft non tender uterus (placental previa)
SFH smaller than age (IUGR from recurrent hemorrhage placental previa)
Malpresentation (placenta previa displace presenting part)
Auscultation: fetal heart absent (placental abruption)
Pelvis
Vaginal examination
Speculum examination done after u/s has excluded placental previa
Visualise the cervix and exclude cervical pathology
Cervical dilatation (placental abruption)
Investigation
Hematology
Maternal
FBC baseline Hb
Group and cross-match
Coagulation profile
Kleihauer test in rhesus negative women
Fetal
CTG deceleration, fetal bradycardia
Ultrasound fetal size, presentation, amniotic fluid, placental position
Ultrasound
